经转子病灶刮除加自体骨膜骨颗粒植入治疗股骨头缺血性坏死疗效观察

CLINICAL STUDY ON TREATMENT OF AVASCULAR NECROSIS OF FEMORAL HEAD BY CORE FOCUS CURETTEMENT AND IMPLANTATION OF AUTOGENOUS PERIOSTEUM AND BONE GRANULES THROUGH TROCHANTER

为探讨经转子病灶刮除加自体骨膜骨颗粒植入治疗股骨头缺血性坏死的临床疗效。经转子髓心钻孔减压 2 9例、经转子髓心减压加自体骼骨植入 30例、经转子病灶刮除加自体骨膜骨颗粒植入 32例。随访 1～ 9年 ,比较 3种方法疗效。结果显示 ,第3组髋关节功能及 X线表现评定与前两组比较经 Ridit检验有显著差异 (P<0 .0 1)。提示经转子病灶刮除加自体骨膜骨颗粒植入治疗股骨头缺血性坏死疗效显著。

Objective: To study the effects of avascular necrosis of femoral head treated by core focus curettement and implantation of autogenous periosteum and bone granules through trochanter. Methods: The first group(29 cases) were made decompression of medullary cavity through trochanter, the second group(30 cases) were made decompression of medullary cavity through trochanter and bone grafting from ilium, the third group(32 cases) were made core focus curettement and implantation of autogenous periosteum and bone granules through trochanter. The therapy of three methods were compared after 1~9 years. Results: The function and X-ray of hip joint of third group were the best in three groups and there was significant difference(P<0.01). Conclusion: The therapeutic effects of avascular necrosis of femoral head treated by core focus curettement and implantation of autogenous periosteum and bone granules through trochanter is obvious.
