摘要
目的 探讨体感诱发电位 (somatosensoryevokedpotentials,SEPs)在评价脑卒中急性期患者肢体运动功能中的作用 ,及其与临床评估在预测脑卒中后肢体运动功能恢复中的价值。方法 脑卒中急性期患者 2 5例 ,在发病 7d内和住院 30d时分别进行上、下肢SEPs检测 ,同时进行上、下肢Fugl Meyer评分 (F M评分 ) ,对照组 1 8例发病 7d内进行上、下肢SEPs检测 ,比较两组患者SEPs的潜伏期 ,并将病例组在发病 7d内和住院 30d时的SEPs潜伏期与F M评分进行相关分析 ;根据病例组是否存在感觉障碍 ,分成有和无感觉障碍组 ,比较两组潜伏期的变化。结果 病例组和对照组上、下肢SEPs潜伏期差异有显著性意义 ,病例组入院时和住院 30d时SEPs潜伏期 (除P1 4、N30 、P2 2 、N35)与上肢 (包括腕、手 )和下肢F M评分有相关性。有感觉障碍组上、下肢SEPs和各电位潜伏期较无感觉障碍组普遍延长。结论 脑卒中急性期患者 ,尤其是存在感觉障碍者SEPs潜伏期的改变较为敏感 ,并与F M评分有相关性 。
Objective To study the role of somatosensory evoked potentials (SEPs) combined with Fugl Meyer (F M) scores in evaluating motor function and prognosis of patients with acute stroke Methods SEPs were measured in 25 patients with acute stroke, who were recruited as subjects for the experimental group, in 7 days after onset and 30 days after hospitalization. The F M scores were assessed at the same time. SEPs that were measured in 18 patients 7 days after the onset served as control. The value of latency of SEPs was compared between the two groups. Correlation between the SEPs and F M scores was analyzed in these patients. Patients in the experimental group were again divided into two groups according to whether the paresthesia was present or not. The value of latency of SEPs was also compared between the two groups Results The value of latency of the SEPs between the experimental and control groups were significantly different and closely correlated with F M scores in most of the patients Conclusion The latency value of SEPs may be a sensitive parameter in the evaluation of motor functional recovery in patients with acute stroke The changes of SEPs seem to be significantly correlated with the F M scores of these patients
