全身骨显像联合血清CA15-3、CEA、ALP对乳腺癌骨转移的诊断价值 被引量:17

摘要 目的通过全身骨显像(WBS)联合糖类抗原(CA15-3)、癌胚抗原(CEA)及血清碱性磷酸酶(ALP)的检测,探讨其监测、辅助诊断乳腺癌骨转移的临床意义。方法对105例经病理证实为乳腺癌且进行全身骨显像的患者,同时进行血清CA15-3、CEA、ALP的检测。结果乳腺癌骨转移组血清CA15-3、CEA、ALP水平明显高于未转移组(t=6.798、5.041、6.146,P<0.05)。在骨转移组中,随骨转移分级升高,血清学表现呈显著上升趋势,全身骨转移分级与CA15-3、CEA、ALP水平存在相关性(r=0.685、0.618、0.625,P<0.05)。全身骨显像联合血清CA15-3、CEA、ALP在诊断乳腺癌骨转移中的敏感度性高于单独检测及二联、三联检测。结论全身骨显像联合血清CA15-3、CEA、ALP的检测,可以提高乳腺癌骨转移诊断的敏感性,可以提高乳腺癌骨转移的检出率。 Objective To investigate the clinical significance of whole body bone scintigraghy(WBS)combined with CA15-3,CEA and ALP in the detection of bone metastasis in breast cancer.Methods Serum CA15-3,CEA and ALP were detected in 105 patients with pathologically confirmed breast cancer who underwent WBS.Results Serum CA15-3,CEA and ALP levels in breast cancer bone metastasis group were significantly higher than those in non-metastatic group(P<0.05).Serum CA15-3,CEA and ALP levels in the multiple bone metastases group were significantly higher than those in the single bone metastases group(P<0.05).With the increase of bone metastasis grade,serology showed a significant upward trend;and systemic bone metastasis grade was correlated with CA15-3,CEA and ALP levels(r=0.685,0.618 and 0.625,respectively;P<0.05).The sensitivity of WBS combined with serum CA15-3,CEA and ALP in the diagnosis of bone metastasis of breast cancer is significantly higher than that of detecting tumor markers alone.Conclusion WBS combined with Serum CA15-3,CEA and ALP can improve the sensitivity of bone metastasis diagnosis and the detection rate of bone metastasis in breast cancer.
