小儿松果体区生殖细胞瘤的治疗

Treatment of intracranial pineal germinomas in children

目的 对经不同手术方法治疗的松果体区生殖细胞瘤的患儿进行分析 ,比较肿瘤全切除组与肿瘤部分切除和活检组患儿的近期疗效及并发症情况。方法 回顾性分析 32例松果体区生殖细胞肿瘤患儿进行显微手术治疗的结果、切除肿瘤程度 ,脑积水缓解及随访生存时间。结果 肿瘤全切除 2 4例 ,肿瘤部分切除和活检 8例 ,两组病例 4年存活率差异无显著性意义 (P >0 .0 5 )。肿瘤全切除患儿的脑积水消除要优于肿瘤部分切除和活检 (P <0 .0 5 )。结论 小儿松果体区生殖细胞肿瘤应尽可能全切除 。

Objective To compare results of total resection and partial removal with biopsy in children with pineal tumor. Methods A retrospective review of 32 pediatric patients with pineal tumors were carried out. The extent of tumor resection, relieve of hydrocephalous and survival rates were analyzed. Results There was no significant difference in 4 year survival rates between two groups ( P > 0.05 ), but there was significant difference in the need of CSF shunt between the two groups ( P < 0.05 ). Conclusion Total resection of pineal germinoma is better than partial resection for the relief of the hydrocephalous.
