吻合口溃疡的临床研究

Clinical study on stomal ulcer of gastroduodenal benign diseases after operation

目的 探讨吻合口溃疡的发生原因、诊断及预后。方法 对胃十二指肠良性疾病术后吻合口溃疡的一般资料 ,胃镜、组织病理、X线检查结果进行相关性研究。结果 有手术方式记录的 43例中 ,毕氏Ⅱ式占 67.4% ,毕氏Ⅰ式占 3 2 .6% ;胃镜下胆汁返流 3 1例 ,其中毕氏Ⅱ式占 80 .6% ,毕氏Ⅰ式占 19.4% ;组织病理 :残胃侧均有不同程度的炎症 ,5例为萎缩性胃炎 ,6例合并肠上皮化生 ;X线检查诊断率为 5 2 .9%。结论 吻合口溃疡的发生主要与肠粘膜屏障功能差 ,吻合口处血运差 ,胆汁返流 ,胃切除不足有关 。

Objective To manage the cause of stomal ulcer,diagnosis and prognosis.Methods Study on the relationship among the same as data of stomal ulcer of gastroduodenal benign diseases after operation,grastroscopy,histopathology and X-ray.Results Among the 43 cases of operation records,Billroth Ⅱ67.4%,Billroth Ⅰ32.%;bile regurgitation 31 below gastroscopy,Billroth Ⅱ 80.6%,Billroth Ⅰ 19.4%;histopathology:There're different extend inflamation in residual stomach,atrophic gastritis 5 cases,intestinal metaplasis 6 cases,diagnosis rate of X-ray 52.9%.Condusions Occur rate of stomal ulcer is retaled with the function of protective screen of intestinal mucous in lower,so as anastomsis hemoperfusion,Bile regurgitation and gastrectomy lose.It is necessary to observe with gastroscopy regularly.
