经腹与经腹膜外途径腹腔镜下手术治疗前列腺癌的临床比较

Clinical comparison of transabdominal and extraperitoneal laparoscopic surgery in the treatment of prostate cancer

目的探究经腹腔与经腹膜外途径腹腔镜下手术治疗前列腺癌的临床疗效差异。方法回顾性分析我院于2014年1月至2018年12月期间收治的45例前列腺癌患者的临床资料,根据治疗方法的不同将其分为对照组(n=20)与观察组(n=25),对照组患者行经腹腔途径腹腔镜手术治疗,观察组患者行经腹膜外途径腹腔镜手术治疗,比较两组患者临床指标、围术期指标、术后切缘阳性率以及术后控尿效果。结果观察组患者手术时间、出血量、导尿管留置时间、肛门排气时间及住院时间等围术期指标均少于对照组,差异有统计学意义(P<0.05);两组患者术后术后切缘阳性率及控尿效果差异无统计学意义(P>0.05)。结论两种途径手术对前列腺癌患者的临床疗效具有一致性,但是经腹膜外腹腔镜手术方案相比经腹腔途径手术具有用时短、出血量少、导尿管留置、排气时间短且可缩短住院时间等优势。

Objective To investigate the clinical efficacy of laparoscopic and transperitoneal laparoscopic surgery for prostate.Methods The clinical data of 45 patients with prostate cancer admitted to our hospital from January,2014 to December,2018 were retrospectively analyzed.According to different treatment methods,they were divided into control group(n=20)and observation group(n=25),patients in the control group underwent laparoscopic laparoscopic surgery.The patients in the observation group underwent extraperitoneal laparoscopic surgery.The perioperative parameters,positive margin rate and postoperative urinary control were compared between the two groups.Results Perioperative indexes such as operation time,bleeding volume,catheter indwelling time,anal exhaust time and hospitalization time were lower in the observation group than in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in postoperative positive margin rate and urine control effect(P>0.05).Conclusion The clinical efficacy of the two approaches for prostate cancer patients is consistent,but the peritoneal laparoscopic surgery has shorter duration,less bleeding,catheter indwelling,and shorter exhaust time than abdominal surgery.
