CYFRA21-1、Granulysin、miR-17-5p和miR-20a联合对早期鼻咽癌的诊断价值

Diagnostic value of CYFRA21-1,Granulysin,miR-17-5p and miR-20a in early nasopharyngeal carcinoma

目的探讨细胞角蛋白19片段、Granulysin、miR-17-5p和miR-20a联合对早期鼻咽癌的诊断价值。方法选取早期鼻咽癌患者70例为鼻咽癌组,鼻咽炎患者70例为鼻咽炎组,同时选取健康体检者70例为对照组,检测3组患者血清中CYFRA21-1、Granulysin、miR-17-5p和miR-20a的表达量。用ROC曲线下面积分析CYFRA21-1、Granulysin、miR-17-5p和miR-20a联合对早期鼻咽癌的诊断价值。结果早期鼻咽癌患者血清中的CYFRA21-1的表达量和miR-17-5p和miR-20a的相对表达量均明显高于鼻咽炎患者和健康体检者(P<0.05);但是,Granulysin的表达量明显低于鼻咽炎患者和健康体检者(P<0.05)。诊断早期鼻咽癌患者与鼻咽炎患者CYFRA21-1、Granulysin、miR-17-5p和miR-20a的灵敏度分别为76%、76%、73%,80%;特异度分别为80%、96%、93%,87%。诊断早期鼻咽癌患者与健康体检者CYFRA21-1、Granulysin、miR-17-5p和miR-20a的灵敏度分别为83%、60%、73%、93%;特异度分别为66%、86%、73%、83%。对早期鼻咽癌的联合诊断的灵敏度为86.3%,特异度为94.9%,准确度为91.9%。结论 CYFRA21-1、Granulysin、miR-17-5p和miR-20a可能作为联合诊断早期鼻咽癌的潜在标志物。

Objective To investigate the diagnostic value of cytokeratin 19 fragment(CYFRA21-1),Granulysin,miR-17-5 p and miR-20 a in early nasopharyngeal carcinoma.Methods Seventy patients with early nasopharyngeal carcinoma were selected as nasopharyngeal carcinoma group,70 patients with nasopharyngitis were nasopharyngitis group,and 70 healthy subjects were selected as control group.Three groups of patients were tested for serum CYFRA21-1,Granulysin,miR.The expression levels of-17-5 p and miR-20 a.The area under the ROC curve was used to analyze the diagnostic value of CYFRA21-1,Granulysin,miR-17-5 p and miR-20 a in early nasopharyngeal carcinoma.Results The expression of CYFRA21-1 and the relative expression of miR-17-5 p and miR-20 a in serum of patients with early nasopharyngeal carcinoma were significantly higher than those of nasopharyngitis patients and healthy subjects(P<0.05);however,Granulysin The expression level was significantly lower than that of patients with nasopharyngitis and healthy subjects(P<0.05).For diagnosis the sensitivity of CYFRA21-1,Granulysin,miR-17-5 p and miR-20 a in patients with early nasopharyngeal carcinoma and nasopharyngitis was 76%,76%,73%,80%,respectively;specificity was 80%,96%,93%,87%.For diagnosis the sensitivity of CYFRA21-1,Granulysin,miR-17-5 p and miR-20 a in patients with early nasopharyngeal carcinoma and healthy subjects was 83%,60%,73%,and 93%,respectively;specificity was 66%,86%,73%,83%.The sensitivity of the combined diagnosis of early nasopharyngeal carcinoma was 86.3%,the specificity was 94.9%,and the accuracy was 91.9%.Conclusion CYFRA21-1,Granulysin,miR-17-5 p and miR-20 a can be used as potential markers for the early diagnosis of nasopharyngeal carcinoma.
