社区为本医务社会工作模式探索——以南海桂城“健康到家”医务社工服务项目为例

Exploration of Community-based Medical Social Work Model——Taking the “Healthy Home” Medical Social Work Service Project in Guicheng of Nanhai as an Example

与传统医务社工相比,社区为本医务社工有以下四个特点:1.社工以服务社区为本,服务对象为社区居民;2.社区医务社工服务结合医疗卫生政策开展,包括家庭医生服务和分级诊疗制度;3.社工在社区中构建发展、预防、补救三级医疗服务体系;4.社工通过整合社区资源,为社区居民搭建社区医疗支持网络。社区为本医务社会工作着重于社区内居民群体自我管理健康能力的发掘与拓展,包括搭建社区医疗资源网络以及寻找社区康复资源。社区为本医务社会工作不是传统医务工作在社区中的简单延伸,而是结合国家医疗卫生政策发展,以社区为主体开展医务社工服务。

Compared with traditional medical social workers,community-based medical social workers have the following four characteristics:1.they are community-based and their service objects are community residents;2.community medical social work is integrated with medical and health policies,including family doctor services and graded diagnosis and treatment system;3.social workers build and develop the three-level medical service system of development,prevention and remedy in the community;4.social workers build community medical support network for community residents by integrating community resources.Community-based medical social work focuses on exploring and expanding community residents′abilities to self-manage their health,including building community medical resources network and searching for community rehabilitation resources.Community-based medical social work is not a simple extension of traditional medical work in the community,but a community-based medical social work service combined with the development of national health policies.
