PVP治疗骨质疏松性椎体压缩骨折术后再骨折原因分析 被引量:9

摘要 目的探讨经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩骨折(OVCF)术后再骨折的影响因素。方法分析因OVCF行PVP术后失败21例的临床资料,术后5~10个月内出现不同程度腰部疼痛及活动受限,比较首次住院手术前后及再次住院后伤椎前缘改变、矢状位Cobb角改变,2次住院期间骨密度改变,分析其失败的原因。结果患者首次住院手术前后及再次住院后伤椎前缘高度分别为(0.95±0.08)、(0.96±0.09)、(0.79±0.07)cm,首次手术前后伤椎前缘高度比较,差异无统计学意义(P>0.05),再次住院后伤椎前缘高度与首次术后伤椎前缘高度比较明显减少(P<0.05);患者首次住院手术前后及再次住院后矢状位Cobb角分别为(17.94±1.21)°、(18.01±1.01)°、(24.02±0.94)°,首次手术前后矢状位Cobb角并无明显改变(P>0.05),再次住院后矢状位Cobb角较首次术后矢状位Cobb角明显变大(P<0.05);2次住院期间骨密度分别为-2.9±0.32、-3.4±0.42,再次住院期间骨密度较首次住院期间骨密度明显降低(P<0.05)。结论 OVCF行PVP术后再骨折与患者骨密度变化、脊柱应力改变相关。
