摘要 目的研究PAX-5、CD117、PDGFRA在胃肠道神经内分泌肿瘤中的表达,并分析与肿瘤生物学特点及患者生存时间的关系。方法应用组织芯片及免疫组化法,对96例胃肠道神经内分泌肿瘤的免疫表型和预后进行分析。结果 PAX-5、CD117、PDGFRA蛋白的阳性率分别为31.3%、19.8%、53.1%。PAX-5和PDGFRA表达与胃肠道神经内分泌肿瘤类型、肿瘤大小、TNM分期及淋巴结转移有显著相关性,CD117表达与TNM分期及淋巴结转移有显著相关性。TNM分期、淋巴结转移、肿瘤大小、CD117、P53、Ki-67表达与生存时间呈负相关,但只有TNM分期、CD117表达和Ki-67高表达为影响预后的独立因素。结论 PAX-5、CD117、PDGFRA的表达可能与胃肠道神经内分泌肿瘤的发生发展有关,CD117表达和Ki-67高表达是独立预后因子,PAX-5可以作为潜在的分子治疗靶点。 Objective To investigate the expression of PAX-5,CD117and PDGFRA in gastrointestinal neuroendocrine neoplasm(GINEN)and their relationship to tumor biological characteristics and survival time of patients.Methods Ninety-six cases in tissue microarray were studied by immunohistochemistry.The relationship between the expression of PAX-5,CD117,PDGFRA and the clinicopathological factors was analyzed by Chi-square test,multivariate analysis and Kaplan-Meier method.Results The positive expression rate of PAX-5,CD117and PDGFRA in GINEN was 31.3%,19.8%and 53.1%.Both PAX-5and PDGFRA related to pathological type,tumor size,TNM stage,and lymph node metastasis(P<0.05);while CD117related to TNM stage and lymph node metastasis(P<0.05).Besides the clinicopathological factors including TNM stage,lymph node metastasis and tumor size,the expression of CD117,P53and Ki-67also showed negative relationships to prognosis in Kaplan-Meier survival analysis(P<0.05);but Cox survival analysis demonstrated that only TNM stage,CD117expression and high Ki-67index were associated with survival time of patients(P<0.05).Conclusion The expression of PAX-5,CD117and PDGFRA plays important roles in the pathogenesis of GINEN.Both CD117expression and high Ki-67index are independent prognostic factors.PAX-5could be a potential target for therapy.
