慢性心力衰竭患者延续护理的研究现状

The present research progress of transitional care for patients with Chronic Heart Failure: a literature review

慢性心力衰竭为各类心脏疾病的终末阶段,随着心血管疾病的发病率升高,现存及潜在的慢性心力衰竭患者也随之增加。该病有易反复发作的特点,延续护理作为控制疾病发作及进展的重要手段,对患者的康复意义重大。本文对慢性心力衰竭延续护理的开展模式进行综述,以期为今后开展相关研究提供参考依据。

Chronic Heart Failure is the outcome of all kinds of heart diseases.As the incidence of cardiovascular diseases increases,the number of existing and potential patients with Chronic Heart Failure will increase accordingly.Moreover,Chronic Heart Failure is prone to occur recurrently.Transitional care,as an important means to control the recurrence and deterioration of the disease,is of great significance to the rehabilitation of patients.This article reviewed the patterns of transitional care for patients with Chronic Heart Failure,in order to provide basis for future research.
