盐酸羟考酮注射液用于老年股骨转子间骨折手术的临床研究

Clinical trial of oxycodone hydrochloride injection in the operation of intertrochanteric fracture of femur in the elderly

目的观察盐酸羟考酮注射液用于老年股骨转子间骨折手术的临床疗效及安全性。方法将80例老年股骨转子间骨折患者随机分为对照组和试验组,每组40例。于麻醉诱导前5 min,对照组予以等容积0. 9%Na Cl,静脉注射;试验组予以0. 1 mg·kg-1盐酸羟考酮注射液,静脉注射。比较2组患者的视觉模拟评分法(VAS)评分和简易精神状况检查(MMSE)评分,以及药物不良反应的发生情况。结果试验组和对照组术后12 h的VAS评分分别为(1. 86±0. 25)和(2. 35±0. 31)分,术后24 h的VAS评分分别为(2. 21±0. 29)和(2. 98±0. 42)分,差异均有统计学意义(均P <0. 05)。术后7 d,试验组和对照组的MMSE评分分别为(28. 26±1. 15)和(26. 35±0. 94)分,认知功能障碍发生率分别为5. 00%和15. 00%,差异均有统计学意义(均P <0. 05)。2组患者均未发生明显的药物不良反应。结论盐酸羟考酮注射液用于老年股骨转子间骨折手术具有良好的镇痛作用,其能有效地改善患者术后认知功能,且安全性较好。

Objective To observe the clinical efficacy and safety of oxycodone hydrochloride injection in the operation of intertrochanteric fracture of femur in the elderly.Methods A total of 80 elderly patients with intertrochanteric fracture of femur were randomly divided into control and treatment groups with 40 cases per group.Five minutes before induction of anesthesia,control group was given intravenous injection of 0.9%Na Cl with equal volume,while the treatment group was given intravenous injection of 0.1 mg·kg-1 oxycodone hydrochloride injection.The visual analogue scale(VAS),mini-mental state examination(MMSE)scores and adverse drug reactions were compared between two groups.Results At 12 hours after operation,the VAS scores of treatment group and control group were(1.86±0.25)and(2.35±0.31),and at 24 hours after operation,the VAS scores were(2.21±0.29)and(2.98±0.42),the differences were statistically significant(all P<0.05).On the 7 th day after operation,the MMSE scores of treatment group and control group were(28.26±1.15)and(26.35±0.94),and the incidences of cognitive impairment were 5.00%and 15.00%,the differences were statistically significant(all P<0.05).No adverse drug reactions were observed in two groups.Conclusion Oxycodone hydrochloride injection has a good analgesic effect and safety in elderly patients with intertrochanteric fracture,which can effectively improve the cognitive function of patients after operation.
