摘要
目的探讨基于基因检测及国际标准化比值(INR)的儿童华法林剂量调整方法。方法通过分析1例肾病综合征合并肺栓塞的患儿应用华法林的治疗过程,整体考虑儿童应用华法林剂量调整的影响因素,根据患儿INR值调整华法林剂量。结果与结论该患儿经过华法林剂量调整后INR值稳定,随后门诊监测调整用药。临床药师应结合患者个体情况调整用药方案,保证用药安全、有效。
Objective To explore the method of adjusting warfarin dose in children by genetic test results and international standardized ratio(INR).Methods By analyzing the treatment process of warfarin in a child with nephrotic syndrome complicated with pulmonary embolism,the influencing factors of dose adjustment in the application of warfarin in children were discussed.Result and conclusion After warfarin dosage adjustment,the INR value of the child was stable,and then the medication was adjusted by outpatient monitoring.Clinical pharmacist should adjuste the medication plan according to the individual situation of patients to ensure the safety and effectiveness of medication.
