身痛逐瘀汤预防高龄股骨粗隆间骨折术后下肢深静脉血栓形成

Shentong Zhuyu Decoction in the Prevention of Postoperative Deep Vein Thrombosis Formation in the Elderly Intertrochanteric Fracture

目的探讨身痛逐瘀汤预防高龄股骨粗隆间骨折术后下肢深静脉血栓形成的临床疗效。方法将80例老年股骨粗隆间骨折术后患者随机分为治疗组41例、对照组39例。治疗组采用身痛逐瘀汤口服加外用,同时皮下注射低分子肝素钠预防下肢深静脉栓塞,对照组采用皮下注射低分子肝素钠进行预防下肢深静脉血栓。通过比较D-二聚体、APTT、PT、PLT、伤口引流量、下肢肢体周径的差异,探讨其安全性。结果两组患者凝血化验结果(D-二聚体、APTT、PT、PLT)发生率无明显差异,治疗组在术后伤口引流量指标明显优于对照组,术后当天两组髌下10 cm、髌上10 cm肢体周径无差异;术后第1天到术后第12天,实验组踝上5 cm、髌下10 cm、髌上10 cm肢体周径显著低于对照组。结论身痛逐瘀汤可有效预防高龄股骨粗隆间骨折术后下肢深静脉血栓形成。

Objective To discuss the clinical effect of Shentong Zhuyu decoction in the prevention of postoperative deep vein thrombosis formation in the elderly intertrochanteric fracture. Methods 80 cases of postoperative deep vein thrombosis formation in the elderly intertrochanteric fracture patients were randomly divided into the treatment group( 41 cases) and the control group( 39 cases). The treatment group was given modified Buyang Huanwu decoction by oral and external application and was given subcutaneous low molecular weight heparin( LMWH). The control group was given subcutaneous LMWH. The safety was evaluated by comparing the differences in Ddimer,APTT,PT,PLT,amount of wound drainage and limb circumference. Results There were no significant differences in D-dimer,APTT,PT,PLT,while amount of wound drainage in the treatment group were obviously better than those in the control. There were no significant differences in limb circumference after operation. There were significant differences in limb circumference from 1thto 12 thday after operation between the two groups. Conclusion Shentong Zhuyu decoction could effectively prevent postoperative DVT formation following elderly intertrochanteric fracture.
