凉血五花汤加减联合红蓝光照射治疗中重度痤疮临床观察

Clinical Observation on Liangxue Wuhua Decoction Combined with Red and Blue Light Irradiation in the Treatment of Moderate and Severe Acne

目的研究和讨论凉血五花汤加减联合红蓝光治疗中重度痤疮的临床疗效。方法将符合纳入标准的64例患者随机分为观察组和对照组,每组32例患者。观察组采用凉血五花汤加减联合红蓝光治疗,对照组采用红蓝光治疗。观察2组患者于治疗前和8周的临床疗效指标;同时观察比较治疗前后患者的心理评分。结果治疗8周后,根据皮损评分进行疗效评估,观察组的总有效率为93. 75%,显著优于对照组的81. 25%,差异具有统计学意义(P <0. 05)。治疗前,2组患者的中医证候、汉密尔顿抑郁量表评分、汉密尔顿焦虑量表评分、病耻感评分和心理弹性评分相比,差异无统计学意义(P> 0. 05)。治疗8周后,2组患者的中医证候评分、汉密尔顿抑郁量表评分、汉密尔顿焦虑量表评分和病耻感评分均显著低于治疗前,差异具有统计学意义(P <0. 05);观察组患者的中医证候评分、汉密尔顿抑郁量表评分、汉密尔顿焦虑量表评分和病耻感评分均显著低于对照组患者,差异具有统计学意义(P <0. 05)。治疗8周后,2组患者的心理弹性评分均显著高于治疗前,差异具有统计学意义(P <0. 05);观察组患者的心理弹性评分显著高于对照组患者,差异具有统计学意义(P <0. 05)。结论凉血五花汤加减联合红蓝光治疗中重度痤疮能有效改善患者的症状,对其心理因素也有干预作用,是治疗胃肠湿热型和肝郁气滞血瘀型痤疮安全有效的方法。

Objective To observe and discuss the application of Liangxue Wuhua decoction combined with red and blue light in treating moderate and severe acne,and to analyze the psychological factors existing in this disease. Methods We selected 64 patients as the research subjects,and divided them into two groups,and each group had 32 patients. The observation group was treated with Liangxue Wuhua decoction combined with red and blue light,while the control group was treated with red and blue light. At the 0th and 8th week of the treatment,according to the index of skin lesion and the score of TCM symptom,and the psychological factors of the two groups were compared by Hamilton anxiety scale( Hama) and Psychoelastic scale( CISISC). Results After 8 weeks,the total effective rate of the observation group was 93. 75%,while the control group was 81. 25%. The effective rate of the observation group was superior to the control group( P < 0. 05). Before treatment,there was no significant difference between the two groups on TCM symptoms,Hamd and Hama scores,stigma score and psychoelasticity score( P > 0. 05). After 8 weeks of treatment,the TCM symptom scores,Hamd and Hama scores and stigma scores of the two groups were significantly lower than those before treatment. The difference was statistically significant( P < 0. 05),and the scores of Hamd,Hama and stigma in the observation group were significantly lower than those in the control group( P < 0. 05). After8 weeks of treatment,the scores of psychoelasticity in the two groups were significantly higher than those before the treatment( P < 0. 05),and the scores of the patients in the observation group were significantly higher than those in the control group( P < 0. 05). Conclusion Liangxue Wuhua decoction combined with red and blue light can effectively improve the symptoms and psychological factors of patients with moderate and severe acne. It is a safe and effective method for the treatment of acne( gastrointestinal dampness and heat type and stagnation of qi and blood stasis type of liver).
