牵腕蹬肩法治疗肩关节前脱位166例

Anterior Dislocation of Shoulder Joint Treated by Pushing Wrist and Pushing Shoulder for 166 Cases

目的对比"牵腕蹬肩法"与"手牵足蹬法"闭合复位治疗肩关节前脱位患者的疗效。方法将2009年12月—2017年12月骨伤科治疗的266例肩关节前脱位患者作为研究对象,实施回顾性分析,依据采取的治疗方法不同分为牵腕蹬肩组(166例)、手牵足蹬组(100例),比较2组肩关节功能恢复时间、疼痛VAS评分、肩关节功能UCLA评分、临床疗效、复位成功率、并发症发生率。结果 2组患者肩关节功能恢复时间、复位前及复位后VAS评分、肩关节功能恢复UCLA评分状况、临床疗效结果经对比,差异无统计学意义(P>0. 05),牵腕蹬肩组患者1次复位成功率高于手牵足蹬组患者,差异有统计学意义(P <0. 05),牵腕蹬肩组患者并发症发生率低于手牵足蹬组患者,差异有统计学意义(P <0. 05)。结论采取"牵腕蹬肩"法闭合复位治疗肩关节前脱位患者的疗效较"手牵足蹬"法更优,成功率更高,安全性更好,存在推广应用价值。

Objective To compare the curative effect of'holding wrist and pushing shoulder method'and'Hippocrates reduction method'in the treatment of anterior dislocation of shoulder joint.Methods From December 2009 to December 2017 in our orthopedics treatment,retrospective analysis of 266 patients with anterior dislocation of shoulder joint research materials was carried out.According to the different treatment methods,they were divided into holding wrist and pushing shoulder group(166 cases)and Hippocrates reduction group(100 cases).The shoulder joint function recovery time,pain VAS score,UCLA score shoulder function,the results of the clinical curative effect,reset the success rate and complication rates was compared.Results The shoulder joint function recovery time,reset before and after the reset VAS score,the condition of shoulder joint function restore UCLA score,the results of the clinical efficacy of contrast,countless between the two groups had no statistical significance(P>0.05).The shoulder once reset the success rate of holding wrist and pushing shoulder group was higher than that of the Hippocrates reduction group,and there had data statistical significance(P<0.05).The incidence of complications in the Hippocrates reduction group was lower than that in the holding wrist and pushing shoulder group,and there had data statistical significance(P<0.05).Conclusion The'holding wrist and pushing shoulder'method combined with the closed reduction method has better effect,higher success rate and better safety than the Hippocrates reduction method,which is worthy of popularization and application.
