益气养阴清热活血法治疗早期糖尿病肾病的疗效及对血液流变学的影响

Discussion on the Curative Effect of Supplementing Qi and Nourishing Yin for Clearing Heat and Activating Blood Circulation Method in the Treatment of Early Diabetic Nephropathy and the Influence on Hemorheology

目的探究益气养阴清热活血法治疗早期糖尿病肾病的疗效及对血液流变学的影响。方法选取2015年1月—2016年7月佳木斯市中医院收治的早期糖尿病肾病患者60例作为研究对象,以抽取红黄方式分组,红球为试验组,黄球为对照组,每组30例,试验组采用常规药物联合益气养阴清热活血法治疗,对照组采用常规药物治疗,分别观察2组患者24 h尿白蛋白排泄率、糖化血红蛋白、餐后2 h血糖、空腹血糖、全血高切黏度、全血低切黏度、纤维蛋白原、红细胞压积、不良反应出现率。结果试验组24h尿白蛋白排泄率、糖化血红蛋白、餐后2 h血糖、空腹血糖、全血高切黏度、全血低切黏度、纤维蛋白原、红细胞压积改善程度优于对照组;不良反应出现率低于对照组(6. 67%<26. 67%),P <0. 05。结论与常规用药相比,早期糖尿病肾病患者加用益气养阴清热活血药方治疗,不仅利于改善血液流变学、血脂指标,更利于降低长期大量使用西药产生的不良反应症状,具有独特的优势。值得临床进一步推广应用。

Objective To explore the curative effect of supplementing qi and nourishing yin for clearing heat and activating blood circulation method in the treatment of early diabetic nephropathy and the influence on hemorheology.Methods From January 2015 to July 2016,60 patients with early diabetic nephropathy treated in our hospital were selected as study objects.The experimental group was treated with routine medicine combined with the method of supplementing qi,nourishing yin and clearing away heat and activating blood circulation,while the control group was treated with routine medicine.The 24 hour urinary albumin excretion rate,glycosylated hemoglobin,postprandial blood glucose and fasting blood glucose,whole blood high shear viscosity,the whole blood low shear viscosity,fibrinogen,hematocrit,adverse reaction rate were observed respectively in the two groups.Results 24 hours urinary albumin excretion rate,glycosylated hemoglobin,2 hours postprandial blood glucose,fasting blood glucose,whole blood high shear viscosity,whole blood low shear viscosity,fibrinogen,hematocrit in the experimental group improved better than the control group.The occurrence rate of adverse reaction of the experimental group was lower than that of control group(6.67%<26.67%)(P<0.05).Conclusion Compared with the routine medication,the treatment of early diabetic nephropathy with the prescription of supplementing qi,nourishing yin and clearing heat and activating blood circulation is not only beneficial to blood change,but also to the improvement of blood lipid index and the reduction of long term.The adverse reaction symptoms caused by extensive use of western medicine have unique advantages.It is worthy of further clinical application.
