中药调剂常见差错的相关原因及其应对措施分析

Relevant Reasons for Common Mistakes in Traditional Chinese Medicine Dispensing and Analysis of Their Countermeasures

目的分析中药调剂常见差错的相关原因及其应对措施。方法回顾性分析2016年3月—2018年9月中药房在中药调剂中出现差错的116例病例资料,分析中药调剂常见差错的相关原因,并提出相应的应对措施。结果本研究中116例出现中药调剂差错的病例中,主要原因包括药品因素、调配因素及处方因素,占比分别为40. 52%、32. 76%、26. 72%。结论中药调剂过程中极易出现差错,应积极对差错形成的原因进行分析,并采取针对性的应对措施,以降低差错发生率,提升用药准确性及安全性。

Objective To analyze the causes of common errors in traditional Chinese medicine dispensing and the corresponding measures.Methods 116 cases of errors in dispensing of traditional Chinese medicine in our hospital from March 2016 to September 2018 were retrospectively analyzed.The causes of common errors in dispensing of traditional Chinese medicine were analyzed,and corresponding countermeasures were put forward.Results Among 116 cases of errors in TCM dispensing,the main reasons were drug factors,dispensing factors and prescription factors,accounting for 40.52%,32.76%and 26.72%respectively.Conclusion Errors are easy to occur in the process of dispensing traditional Chinese medicine.The causes of errors should be analyzed actively,and corresponding measures should be taken to reduce the incidence of errors and improve the accuracy and safety of medication.
