颈型颈椎病中医外治防治方案临床研究

Clinical Study on External Treatment of Traditional Chinese Medicine Prevention and Control Program for Neck Cervical Spondylosis

目的观察中医外治方案对颈型颈椎病的防治效果。方法选取2017年1月—2018年6月收治的颈型颈椎病患者250例作为研究对象,随机分为观察1、2、3、4组和对照组,每组50例。观察1组予推拿手法和功能锻炼,观察2组予中药热敷,观察3组予灸法,观察4组予健康指导,对照组为空白对照。比较各组患者干预前Vernon颈椎残障指数量表(NDI)评分、生活质量指数(SF-36)评分以及颈椎病康复患者复发天数及因颈椎病治疗的费用。结果干预前5组NDI评分、SF-36评分比较无统计学差异,P> 0. 05,干预后推拿和功能锻炼组、中药热敷组、灸法组、健康教育组NDI评分、SF-36评分与干预前比较,差异有统计学意义,P <0. 05;组间比较:推拿和功能锻炼组、中药热敷组、灸法组、健康教育组均优于对照组(P <0. 01),推拿和功能锻炼组、灸法组与中药热敷组、健康教育组比较,差异有统计学意义(P <0. 05)。结论在防治颈型颈椎病时,可将中医外治方法综合应用几种,使防治效果达到最大化。优选推拿手法和功能锻炼、灸法作为颈型颈椎病中医外治综合防治方案,并可配合中药热敷疗法、健康宣教,进一步提高疗效,形成防治结合体系,以控制疾病的复发,提高患者的生活质量。

Objective To observe effect of TCM external treatment and health care for the neck cervical spondylosis.Method 250 cases with the neck cervical spondylosis from January 2017 to June 2018 were randomly divided into five groups(n=50),including four observation groups and one control group.Observation group one was treated by massage manipulation and functional training.Observation group two was treated by TCM fomentation.Observation group three was treated by moxibustion.Observation group four underwent health guidance.The control group was a blank control group.The Neck Disability Index(NDI)score,Short Form(36)Health Survey(SF-36)score,relapse time in recovery patients,and medical cost were recorded and analyzed.Result Before intervention,NDI score and SF-36 score had no statistical difference(P>0.05)among these five groups.NDI score and SF-36 score had statistical difference(P<0.05)between before and after intervened of the four observation groups.Compared with scores of control group,the scores of both four observation groups after intervention were statistically decreased(P<0.01).And after intervention,NDI score and SF-36 score had statistical difference(P<0.05)in four observation groups.Conclusion Treatment combined several of the TCM external therapies could improve effect for the neck cervical spondylosis.Massage manipulation,functional training,and moxibustion were optimized methods for the neck cervical spondylosis.Adding TCM fomentation and health guidance,the effect would be further improved,it’s a system to prevent and treat the neck type cervical spondylosis,and also improve life quality of the patients.
