参皂九味汤治疗子宫内膜异位症临床效果观察及作用机制研究

Observation on the Clinical Effect and Research on the Mechanism of Shenzao Jiuwei Decoction in the Treatment of Endometriosis

目的研究参皂九味汤治疗子宫内膜异位症的效果,分析其对HE4、EMAb和CA125水平的作用。方法选取2017年8月—2018年7月收治的84例子宫内膜异位症患者作为研究对象,按照信封法随机分为对照组(42例)与观察组(42例),对照组采用西药治疗,观察组在其基础上加用参皂九味汤治疗。比较2组的痛经评分、囊肿体积、性激素水平、HE4、EMAb和CA125水平、临床疗效以及不良反应发生情况。结果观察组患者治疗后的痛经评分以及E2、FSH、P水平均低于对照组,囊肿体积小于对照组,差异具有统计学意义(P <0. 05);观察组患者治疗后的HE4、EMAb和CA125水平均明显低于对照组,差异具有统计学意义(P <0. 05);观察组患者的临床总有效率高于对照组,差异具有统计学意义(P <0. 05);2组的不良反应发生率进行比较,差异无统计学意义(P> 0. 05)。结论参皂九味汤应用在子宫内膜异位症患者的治疗中效果确切,改善性激素水平,可对HE4、EMAb和CA125水平进行有效调节,临床应用价值高。

Objective To study the effect of Shenzao Jiuwei decoction on endometriosis and to analyze its effect on HE4,EMAb and CA125 levels.Methods 84 patients with endometriosis admitted from August 2017 to July 2018 were selected as research objects and randomly divided into control group(42 cases)and observation group(42 cases)according to envelope method.The control group was treated with western medicine,and the observation group was additionally treated with Shenzao Jiuwei decoction on the basis.The dysmenorrhea score,cyst volume,sex hormone level,HE4,EMAb and CA125 levels,clinical efficacy and adverse reactions were compared between the two groups.Results The dysmenorrhea score,E2,FSH and P levels of the patients with endometriosis in the observation group were lower than those in the control group,and the cyst volume was smaller than that in the control group.The difference was statistically significant(P<0.05).The levels of HE4,EMAb and CA125 in the observation group were significantly lower than those in the control group after treatment,and the difference was statistically significant(P<0.05).The clinical total effective rate of the patients in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Shenzao Jiuwei decoction is effective in the treatment of patients with endometriosis.It can improve the level of sex hormone,effectively regulate the levels of HE4,EMAb and CA125,and has high clinical application value.
