尪痹胶囊联合小剂量甲氨蝶呤治疗类风湿关节炎临床研究

Clinical Study on Wangbi Capsule Combined with Low Dose Methotrexate in the Treatment of Rheumatoid Arthritis

目的探讨尪痹胶囊联合小剂量甲氨蝶呤治疗类风湿关节炎(RA)的临床疗效及安全性。方法收集RA患者96例,根据患者入院顺序编号,单号者纳入中西医结合组,双号者纳入西医组,每组48例。西医组单纯给予甲氨蝶呤片。中西医结合组则口服甲氨蝶呤片,同时用尪痹胶囊治疗。对比2组治疗前后关节压痛数、关节肿胀数、晨僵时间、红细胞沉降率(ESR)及C-反应蛋白(CRP)、类风湿因子(RF)变化情况。同时对比2组的临床疗效。结果中西医组的总有效率明显高于西医组(P <0. 05)。治疗后,中西医组关节压痛数、关节肿胀数较西医组明显减少,晨僵时间较西医组明显缩短(P <0. 05);ERS、CRP、RF水平较西医组明显降低(P <0. 05)。中西医结合组不良反应总发生率明显低于西医组(P <0. 05)。结论尪痹胶囊联合小剂量甲氨蝶呤治疗RA可有效减轻患者的临床症状,改善实验室指标,减少甲氨蝶呤用量,减轻不良反应,疗效确切且安全性高。

Objective To investigate the clinical efficacy and safety of Wangbi capsule combined with low dose methotrexate in the treatment of rheumatoid arthritis(RA).Methods 96 patients with RA were collected and numbered according to the order of admission.The singlenumbered patients were included in the integrated traditional Chinese and Western medicine group,and the double-numbered patients were included in the western medicine group,with 48 cases in each group.Western medicine group was given methotrexate tablets.In the integrated traditional Chinese and Western medicine group,methotrexate tablets were orally administered,and at the same time,Wangbi capsule was used.The changes of joint tenderness,joint swelling,morning stiffness time,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and rheumatoid factor(RF)before and after treatment were compared between the two groups.At the same time,the clinical effects of the two groups were compared.Result The total effective rate of combination group was significantly higher than that of Western medicine group(P<0.05).After treatment,the number of tenderness and swelling of joints in TCM and WM group were significantly reduced,the time of morning stiffness was significantly shortened compared with WM group(P<0.05),and the levels of ERS,CRP and RF were significantly lower than those in WM group(P<0.05).The total incidence of adverse reactions in the integrated traditional Chinese and Western medicine group was significantly lower than that in the Western medicine group(P<0.05).Conclusion Wangbi capsule combined with low-dose methotrexate in the treatment of RA can effectively alleviate clinical symptoms,improve laboratory indicators,reduce the dosage of methotrexate and adverse reactions,and has definite efficacy and high safety.
