摘要 目的:分析门诊患者人乳头瘤病毒(HPV)的感染情况、年龄特征、性别差异、亚型分布特点,为制定HPV感染的预防策略提供依据。方法:采用实时多重荧光核酸扩增技术,对1 180例门诊就诊者进行HPV基因型的分型检测。结果:330例男性患者,共检出HPV阳性157例,其中高危亚型101例(64.33%);低危亚型106例(67.50%);年龄分布:21~30岁阳性71例(46.41%);31~40岁阳性47例(45.19%);41~50岁阳性29例(50.00%);51岁以上阳性10例(66.67%)。男性患者中检出率最高的3种亚型依次为39型、16型59型。850例女性共检出HPV阳性243例,其中高危亚型198例(81.48%);低危亚型79例(32.51%);年龄分布情况:21~30岁阳性113例(41.09%);31~40岁阳性58例(21.25%);41~50岁阳性43例(24.57%);50~60岁阳性24例(26.09%);61岁以上阳性5例(14.29%)。女性患者中检出率最高的3种亚型依次为58型、16型和52型。结论:女性HPV感染高发于育龄期妇女并呈年轻化趋势,应加大对21~30岁已婚妇女的HPV筛查,同时加大男性HPV感染的筛查对有效降低宫颈癌的发生率和致死率有重要意义。 Objective:To study the characteristics of human papilloma virus (HPV)infection,such as infection rate,age, gender,subtype distribution,in order to provide the basis of its prevention strategies. Methods:The genotypes of HPV in 1 180 outpatients were detected by real-time multiple fluorescent DNA amplification technology. The infectious characteristics was analyzed. Results:There were 157 HPV-positive patients in 330 male outpatients,among them 101 cases (64.33%) were infected high risk subtypes of HPV,106(67.50%)were infected with low-risk subtype of HPV. In the male HPV in-fection patients,age of 71 patients (46.41%)was 21 to 30 years old,47 (45.19%)aged from 31 to 40 years old,29 (50.00%)aged from 41 to 50 years old,and the age of 10 (66.67%)were above 51 years. In the male patients,the infec-tion rate was the highest in type 39,and followed by types 16 and 59. In 850 cases of female outpatients,HPV were found positive in 243 cases,including 198 cases (81.48%)of high-risk subtypes infection and 79 cases (32.51%)of low-risk sub-type infection. In the female HPV positive patients,the age of 113 patients (41.09%)was 21 to 30 years old,58 patients (21.25%)aged from 31 to 40 years old,43 subjects (24.57%)aged from 41 to 50 years old,24 subjects(26.09%)aged from 50 to 60 years old,and 5 persons(14.29%)were above 61 years old. In female patients,type 58 gave rise to the high-est infection rate,followed by type 16 and 52. Conclusions:Women of the child-bearing age have a high infection rate of HPV,and the age of invasion showed a younger tendency. More attention should be paid to screen HPV infection to married women of 21-30 years old. Screening of HPV in male group should be strengthened,and it may be helpful to reduce the inci-dence and mortality of cervical cancer.
