同型半胱氨酸对男性2型糖尿病患者心肌功能及脂代谢的影响

摘要 目的评价2型糖尿病患者同型半胱氨酸(HCY)水平对血脂及心肌功能的影响。方法选择2014年1~10月到该院内分泌科住院的男性2型糖尿病患者140例,分别检测脂代谢功能[三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)]、心肌功能[磷酸肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、肌红蛋白(MYO)]及HCY,依据HCY水平(14μmol/L为临界值)将其分为正常组(72例)和增高组(68例)。结果两组患者心肌功能比较差异有统计学意义(F=15.78,P<0.01),脂代谢功能比较差异也有统计学意义(F=3.18,P=0.03),但影响不明显(Pillai跟踪值为0.65;Hotelling跟踪值为0.70)。结论高HCY水平对心肌功能存在影响,但对患者脂代谢影响有限。 Objective To evaluate the influence of the homocysteine levels on cardiac function and lipid metabolism in patients with type 2diabetes mellitus.Methods A total of 140 male patients with type 2diabetes were randomly enrolled in the study,and were divided into two groups at baseline homocysteine level(more or less than 14μmol/L).Results Multi-variable analysis showed cardiac biomarkers(F=15.78,P<0.001)and lipids(F=3.18,P=0.03)were significantly different in different groups.However,the homocysteine level had a limited impact on lipids(Pillai trace=0.65;Hotelling trace=0.70).Conclusion Elevated homocysteine levels were highly significant in cardiac biomarkers but had a marginal effect on lipids in type 2diabetic patients.
