过敏性紫癜患儿外周血MK水平及其对肾损伤的早期预测价值

The concentration of MK in peripheral blood of children with Henoch Schonlein purpura and its value in prediction of early renal injury

目的分析外周血中期因子(MK)在过敏性紫癜(HSP)患儿中的水平及其对早期肾损伤的预测价值。方法将100例MK患儿纳入本研究,其中早期肾损伤35例,作为A组,无肾损伤65例,作为B组。通过酶联免疫吸附法(ELISA)测定上述人群外周血MK、白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、白细胞介素-17(IL-17)及肿瘤坏死因子-α(TNF-α)水平。另外,选取同期42例健康体检儿童作为对照组。结果 A组外周血MK水平未(321.47±23.85)pg/mL,高于B组的(276.46±28.04)pg/mL以及对照组的(215.28±20.16)pg/mL,差异均有统计学意义(P<0.05)。A组、B组外周血IL-6、IL-17、TNF-α水平均高于对照组(P<0.05),但A、B两组组间比较差异均无统计学意义(P>0.05);Pearson相关性分析发现MK与IL-6、IL-17相关(P<0.05);MK对HSP早期肾损伤预测的敏感度、特异度分别为80.65%、88.42%。结论外周血MK对儿童HSP及其早期肾损伤有一定的预测价值,而且其水平与IL-6、IL-17相关。

Objective To investigate the concentration of midkine(MK)in peripheral blood of children with Henoch Schonlein purpura(HSP)and its value in prediction of early renal injury.Methods Concentrations of MK,interleukin-2(IL-2),interleukin-6(IL-6),interleukin-17(IL-17)and tumor necrosis factorα(TNF-α)in peripheral blood of 100 children were determined by the enzyme-linked immunosorbent assay(ELISA).Children with early renal injury were included in group A(n=35)and those without were included in group B(n=65).Another 42 healthy children were enrolled in the study as control group at the same time.Results MK level in the peripheral blood of group A [(321.47±23.85)pg/mL]was significantly higher than that in group B [(276.46±28.04)pg/mL]and that in the control group [(215.28±20.16)pg/mL](P<0.05).Concentrations of IL-6,IL-17 and TNF-αin peripheral blood of group A and group B were significantly higher than those in the control group(P<0.05),but there was no significant difference between group A and group B(P>0.05).Pearson correlation analysis showed that MK was significantly related to IL-6 and IL-17(P<0.05).The sensitivity and specificity of MK in prediction of early renal injury in HSP were80.65%and 88.42%,respectively.Conclusion Peripheral blood MK is of certain value in prediction of early renal injury in children with HSP,and its level is related to IL-6 and IL-17.
