玉屏风颗粒联合糠酸莫米松鼻喷雾剂治疗慢性鼻炎的临床研究

Clinical study on Yupingfeng Granules combined with Mometasone Furoate Nasal Spray in treatment of chronic rhinitis

目的探讨玉屏风颗粒联合糠酸莫米松鼻喷雾剂治疗慢性鼻炎的临床疗效。方法选取2016年1月—2017年1月开封市祥符区第一人民医院收治的慢性鼻炎患者70例为研究对象,依照入组时间编号并采用随机信封法将其分为对照组和治疗组,每组各35例。对照组吸入糠酸莫米松鼻喷雾剂,2揿/次,1次/d,病情得到控制后剂量减半。治疗组在对照组治疗的基础上温水冲服玉屏风颗粒,1袋/次,3次/d。两组患者均连续治疗4周。观察两组的临床疗效,比较两组的鼻窦CT评分、鼻内镜评分、临床症状评分、免疫功能水平和复发率。结果治疗后,对照组和治疗组的总有效率分别为77.14%、94.29%,两组比较差异有统计学意义(P<0.05)。治疗后,两组鼻窦CT评分和鼻内镜评分显著降低,同组治疗前后比较差异有统计学意义(P<0.05);且治疗组评分指标明显低于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组鼻塞、鼻涕、嗅觉减退、头痛评分均显著降低,同组治疗前后比较差异有统计学意义(P<0.05);且治疗组评分指标明显低于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,两组CD3+、CD4+水平显著升高,而CD8+水平显著下降,同组治疗前后比较差异有统计学意义(P<0.05);且治疗组免疫功能指标水平明显优于对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,对照组和治疗组的复发率分别为17.14%、2.86%,两组比较差异有统计学意义(P<0.05)。结论玉屏风颗粒联合糠酸莫米松鼻喷雾剂治疗慢性鼻炎具有较好的临床疗效,可提高细胞免疫功能,改善临床症状,减少复发率,安全性较好,具有一定临床推广应用价值。

Objective To investigate the effect of Yupingfeng Granules combined with Mometasone Furoate Nasal Spray in treatment of chronic rhinitis. Methods Patients(70 cases) with chronic rhinitis in Xiangfu District First People’s Hospital of Kaifeng from January 2016 to January 2017 were randomly divided into control and treatment groups, and each group had 35 cases. Patients in the control group were inhalation administered with Mometasone Furoate Nasal Spray, 2 presses/time, once daily, and dose was reduced by half after disease control. Patients in the treatment group were po administered with Yupingfeng Granules on the basis of the control group, 1 bag/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacies were evaluated, and sinus CT scores, nasal endoscopy scores, clinical symptom scores, and immune function levels in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 77.14% and 94.29%, respectively, and there was difference between two groups(P < 0.05). After treatment, sinus CT scores and nasal endoscopy scores in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). After treatment, the scores of Nasal congestion, nasal discharge, diminished smell, and headache in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the scores in the treatment group were significantly lower than those in the control group, with significant difference between two groups(P < 0.05). After treatment, the levels of CD3+ and CD4+ in two groups were significantly increased, but the levels of CD8+ in two groups were significantly decreased, and the difference was statistically significant in the same group(P < 0.05). And the immune function levels in the treatment group were significantly better than those in the control group, with significant difference between two groups(P < 0.05). After treatment, the recurrence rates in the control and treatment groups were 17.14% and 2.86%, respectively, and there was difference between two groups(P < 0.05). Conclusion Yupingfeng Granules combined with Mometasone Furoate Nasal Spray has clinical curative effect in treatment of chronic rhinitis, can enhance cellular immune function, improve clinical symptoms, and reduce recurrence rate, with good safety, which has a certain clinical application value.
