知柏地黄丸联合替硝唑治疗牙周炎的临床研究

Clinical study on Zhibai Dihuang Pills combined with tinidazole in treatment of periodontitis

目的探讨知柏地黄丸联合替硝唑胶囊治疗牙周炎的临床效果。方法选取2016年9月—2018年9月青岛市第五人民医院收治的牙周炎患者76例,随机分成对照组(38例)和治疗组(38例)。对照组口服替硝唑胶囊,2粒/次,1次/d。治疗组在对照组基础上口服知柏地黄丸,1丸/次,2次/d。两组患者均连续治疗1周。观察两组患者临床疗效,同时比较治疗前后两组患者牙周组织检查相关指标、龈沟液白介素-1(IL-1)、肿瘤坏死因子-α(TNF-α)和前列腺素E2(PGE2)水平及疼痛视觉模拟评分法(VAS)评分和14项口腔健康影响程度量表(OHIP-14)评分。结果治疗后,对照组临床有效率为73.9%,显著低于治疗组的90.9%,两组比较差异具有统计学意义(P<0.05)。治疗后,两组菌斑指数(PLI)、龈沟出血指数(SBI)较治疗前均显著降低(P<0.05),探诊深度(PD)和附着丧失(AL)亦均显著减小(P<0.05),且治疗组患者PLI、SBI、PD和AL明显小于对照组(P<0.05)。治疗后,两组龈沟液中IL-1、TNF-α和PGE2浓度均显著降低(P<0.05),且治疗组患者IL-1、TNF-α和PGE2浓度明显低于对照组(P<0.05)。治疗后,两组疼痛VAS评分和OHIP-14评分均显著下降(P<0.05),且治疗组患者VAS和OHIP-14评分明显低于对照组(P<0.05)。结论知柏地黄丸联合替硝唑胶囊治疗牙周炎能有效改善患者牙周组织状况,减轻疼痛,控制病情进展,提高生活质量,具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Zhibai Dihuang Pills combined with tinidazole in treatment of periodontitis.Methods Patients(76 cases)with periodontitis in Qingdao Fifth People′s Hosptial from September 2016 to September 2018 were randomly divided into control(38 cases)and treatment(38 cases)groups.Patients in the control group were po administered with Tinidazole Capsules,2 grains/time,once daily.Patients in the treatment group were po administered with Zhibai Dihuang Pills on the basis of the control group,1 pill/time,twice daily.Patients in two groups were treated for 1 week.After treatment,the clinical efficacy was evaluated,and the related indexes of periodontal tissue examination,the IL-1,TNF-αand PGE2 levels in gingival crevicular fluid,VAS scores and OHIP-14 scores in two groups before and after treatment were compared.Results After treatment,the clinical efficacy in the control group was 73.9%,which was significantly lower than 90.9%in the treatment group,and there were differences between two groups(P<0.05).After treatment,the PLI,SBI,PD,and AL in two groups were significantly decreased(P<0.05),and which in the treatment group were significantly lower than that in the control group(P<0.05).After treatment,the IL-1,TNF-α,and PGE2 levels in gingival crevicular fluid in two groups were significantly decreased(P<0.05),and which in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,the VAS and OHIP-14 scores in two groups were significantly decreased(P<0.05),and these scores in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion Zhibai Dihuang Pills combined with tinidazole in treatment of periodontitis can effectively improve the periodontal tissue condition,relieve pain,control disease progress,improve the quality of life,which has a certain clinical application value.
