摘要
为了分析慢性中耳炎细菌学动态及在细菌生物膜形成中的作用,本研究选取我院在2013年12月至2016年12月收治的300例慢性中耳炎患者作为研究对象。选取160例纳入慢性化脓性中耳炎组和140例纳入中耳胆脂瘤组,对中耳分泌物进行细菌培养,分析中耳分泌物的细菌学分布状况和动态变化情况。采用SPSS 20.0统计学软件进行统计学分析不同时间段的主要致病菌分布变化情况、分析慢性中耳炎细菌生物膜形成与细菌培养结果之间的关系、分析细菌生物膜形成患者的病原菌分布进行排序。结果显示,300例患者中共检出240耳病原菌,占80.00%(240/300);160例纳入慢性化脓性中耳炎组检出率与140例纳入中耳胆脂瘤组检出率比较无统计学意义(p>0.05);慢性化脓性中耳炎组和中耳胆脂瘤组病原菌中所占比例最多的是金黄色葡萄球菌,其次是绿脓杆菌、表皮葡萄球菌,两组患者的病原菌构成比总体比较无统计学意义(p>0.05);2013年12月至2014年12月检出率显著高于2015年1月至2016年11月,数据比较有统计学意义(p<0.05)。本研究表明,金黄色葡萄球菌是慢性中耳炎患者的主要病原菌。
In order to analyze the bacteriological dynamics of chronic otitis media and its mechanism in the formation of bacterial biofilm.This study selected 300 patients with chronic otitis media admitted to our hospital from December 2013 to December 2016 as the cases of research.Bacterial culture of Chronic otitis media(160 cases included in the chronic suppurative otitis media group and 140 cases included in the middle ear cholesteatoma group)was carried out to analyze the distribution and dynamic changes of middle ear secretion.SPSS 20.0 statistical software was used to analyze the distribution of major pathogens in different time periods.The relationship between bacterial biofilm formation and bacterial culture was analyzed,and the distribution of pathogens in bacterial biofilm formation was analyzed in this research.The results showed that total of 240 ear pathogens were detected in 300 patients,accounting for 80.00%(240/300).There was no statistical significance(p>0.05)between 160 cases included in the chronic suppurative otitis media group and 140 cases included in the middle ear cholesteatoma group.The highest proportion of pathogens in the chronic suppurative otitis media group and the middle ear cholesteatoma group was Staphylococcus aureus,followed by Pseudomonas aeruginosa and Staphylococcus epidermidis.The pathogen composition in the two groups was not statistically significant(p>0.05).The detection rate from December 2013 to December 2014 was significantly higher than that from January 2015 to November 2016,which had statistically significance(p<0.05).This study showed that Staphylococcus aureus should be the main pathogen of patients with chronic otitis media.
