彩色多普勒超声对2型糖尿病下肢动脉病变的诊断价值

The diagnostic value of color Doppler ultrasound on the lower extremity arterial lesions in type 2 diabetic mellitus

目的:分析彩色多普勒超声对2型糖尿病(DM)下肢动脉病变的诊断价值。方法:选取2017年9月至2018年9月在广西医科大学第一附属医院内分泌科住院确诊的2型DM患者69例为DM组,同期入院的50例非2型DM患者为非DM组。通过彩色多普勒超声分别检测两组股总、腘、足背动脉管腔内径(D)、收缩期峰值流速(PSV)、一个心动周期血流量(BFV),比较两组足背与股总、腘动脉的血流动力学参数、PSV和BFV比值差异。结果:DM组足背动脉D、BFV值小于非DM组,腘、足背动脉PSV值均大于非DM组,足背与股总、腘动脉BFV比值均小于非DM组,差异均有统计学意义(均P<0.05)。结论:足背与股总、腘动脉BFV比值可客观、分节段地反映2型DM下肢动脉病变程度,彩色多普勒超声在2型DM下肢动脉病变诊断中具有重要的临床价值。

Objective:To evaluate the diagnostic value of color Doppler ultrasound on the lower extremity arterial lesions in type 2 diabetic mellitus(DM).Methods:From September 2017 to September 2018,69 patients with DM(DM group)and 50 patients without DM(non-DM group)treated in our hospital were enrolled.The following parameters were tested and compared between the two groups:interior diameter(D),peak systolic velocity(PSV),blood flow volume(BFV)of bilateral common femoral,popliteal and dorsalis pedis arteries.Results:The D and BFV of dorsalis pedis arteries,the ratio of dorsalis pedis artery BFV to common femoral artery BFV,and the ratio of dorsalis pedis artery BFV to popliteal artery BFV in DM group were lower than those in non-DM group,while the PSV of popliteal artery and dorsalis pedis artery were higher(P<0.05).Conclusion:Color Doppler ultrasound was valuable for the diagnosis of lower extremity arterial lesions in type 2 DM patients.The ratio of dorsalis pedis artery BFV to common femoral artery or popliteal artery BFV could objectively and segmentally reflect the lower extremity arterial lesions in patients with type 2 diabetes.
