摘要
目的:探讨中医护理干预对缺血性脑卒中后上肢痉挛患者肢体功能的影响。方法:将60例缺血性脑卒中伴上肢痉挛患者按随机数字表法随机分为试验组(30例)与对照组(30例)。两组患者均接受缺血性脑卒中的内科治疗和康复训练,接受缺血性脑卒中护理常规与健康教育,试验组在此基础上加中医护理干预,连续干预30天。采用改良Ashworth痉挛量表、简化Fugl-Meyer运动功能量表及临床神经功能缺损量表对两组患者进行评定。结果:干预后试验组的改良Ashworth痉挛评级综合疗效高于对照组,差异具有统计学意义(P<0.05);简化Fugl-Meyer运动功能评分明显高于对照组,差异具有统计学意义(P<0.05);临床神经功能缺损评分低于对照组,差异具有统计学意义(P<0.05)。结论:中医护理干预结合康复训练能缓解脑卒中后肢体痉挛,改善脑卒中后上肢痉挛患者的运动功能,降低患者的神经功能缺损,其疗效明显优于单纯的康复治疗。
Objective:To observe the influence of Traditional Chinese Medicine(TCM)nursing intervention to muscle tension, motor function and neurologic impairment on patients with upper limb spasm after ischemic stroke. Methods:60 old patients of the upper limb spasticity after stroke were recruited, and they were randomly divided into test group(30 cases) and control group(30 cases) by the randomized digital table. All patients received routine treatment and health education for 30 days, while the patients of treatment group received TCM nursing. The changes of muscle tension, motor function and neurologic impairment in the two groups during the intervention period were recorded and evaluated. Results: The Ashworth spasm rating, the Fugl-Meyer Assessment index in treatment group presents a significant decrease in comparison with that in the control group after treatments(P<0.05). The CSS in treatment group presents a significant decrease in comparison with that in the control group after treatments(P<0.05). Conclusions: TCM nursing combined with rehabilitation therapy can alleviate limb spasm after stroke, improve motor function and can reduce the neurological deficits for patients, its curative effect is obviously superior to the single.
