极低出生体重儿前后五年救治情况比较

IMPROVEMENT IN THE SURVIVAL RATE OF VERY LOW BIRTHWEIGHT IN- FANTS

通过总结1997年1月至2001年12月我院儿科NICU极低出生体重儿(VLBWI)的收治情况,并与前5年进行比较,结果显示VLBWI收治数增加;呼吸系统疾病发生率降低,如窒息的发生率自62.1％减少到30％,肺透明膜病的发生率自72.4％减少到58.3％;应用肺表面活性物质、高频振荡通气、中心静脉插管等新技术从17.2％增加到86.7％,从而使治愈率自73.9％提高到89.1％,平均住院日自46.8天缩短到43.3天。治疗体会包括加强孕期保健,产儿科合作进行复苏,积极防治呼吸系统疾病,维持内环境稳定,防治院内感染,保证营养需求和提高医护质量。

To study the outcome of very low birthweight infants (VLBWI), clinic information of 60 cases of VLBWE in the neonatal intensive care unit (NICU) in Peking Union Medical College Hospital during January 1997 to December 2001 was analyzed. It was showed that the number of VLBWI was increased; the morbidity of respiratory diseases was reduced, such as asphyxia from 62.1% to 30% , NRDS from 72.4% to 58.3%; the application of new therapies, e.g. surfactant, HFOV, central venous catheter was increased from 17.2% to 86.7% ; the survival rate was increased from 73.9% to 89.1% ; the time hospitalization was shorted from 46. 8 to 43.3 days as compared with those VLBWI during 1992 to December 1996. It was important to pay attention to pregnant health care, resuscitation in delivery room in cooperation with pediatricians, prevention and treatment of respiratory diseases, stabilizing internal condition , prevention and treatment of nosocomial infection, meeting nutritional requirement and improvement of medical and nurse level.
