摘要
目的:探讨卵巢形态学相关超声参数在多囊卵巢综合征(PCOS)诊断中的应用价值。方法:选取2015年1月至2017年6月于我院妇产科门诊接受诊治的38例PCOS患者以及同期的38例正常育龄妇女为研究对象,两组患者均接受经阴道超声检查,并在超声图像上测量卵巢形态学相关参数,包括卵泡数量(FN)、卵巢体积(OV)、卵巢间质面积(SA)、卵巢总面积(TA)以及SA/TA比值,观察并比较两组患者卵巢形态学超声参数之间的差异。结果:PCOS组患者FN、OV、SA以及SA/TA比值与对照组相比显著升高(P<0.05)。PCOS组患者血清LH浓度以及LH/FSH比值显著高于对照组患者(P<0.05)。此外,PCOS组患者形态学参数FN、SA、SA/TA与血清学参数LH、LH/FSH之间有良好的相关性,其中SA/TA与LH/FSH之间相关性最好(r=0.78,P<0.05)。结论:卵巢形态学超声参数在诊断PCOS中有较好的临床应用价值,其中SA/TA诊断PCOS的价值最高。
Objective: To investigate the clinical value of ovarian morphology-related ultrasound parameters in the diagnosis of polycystic ovary syndrome( PCOS). Methods: Thirty-eight PCOS patients treated in the Department of Ultrasound of our hospital between January 2015 and June 2017,and a contemporary cohort of 38 women of normal reproductive age were included as the subjects in the study. Both groups received transvaginal ultrasonography. The ovarian morphology-related ultrasound parameters on the ultrasound images were measured,including follicle number( FN),ovarian volume( OV),ovarian interstitial area( SA),total ovarian area( TA),and SA/TA ratio. The differences in the ovarian morphology-related ultrasound parameters were determined and compared between the two groups. Results: The ratios of FN,OV,SA,and SA/TA ratio in the PCOS group significantly increased compared with those in the control group,with statistical significance( P < 0. 05). The serum LH concentration and LH/FSH ratio in the PCOS group were significantly higher than those in the control group,with statistical differences. In addition,there were good correlation between morphological parameters( FN,SA and SA/TA) and serological parameters( LH and LH/FSH) in the PCOS group,among which SA/TA and LH/FSH showed the best correlation( r = 0. 78,P < 0. 05). Conclusion: Ovarian morphology-related ultrasound parameters show good clinical value in the diagnosis of PCOS,among which SA/TA shows the highest value.
