颈源性头痛相关因素的多元Logistic回归分析

Multivariate logistic regression analysis of factors related to cervicogenic headache

目的:调查颈源性头痛患者与非颈源性头痛患者间存在的差异因素,分析颈源性头痛的独立危险因素。方法:分别对198例颈源性头痛患者和80例非颈源性头痛患者进行差异性分析,采用多因素Logistic回归法分析颈源性头痛的独立危险因素。结果:颈源性头痛患者组与非颈源性头痛患者组在年龄、性别、每日伏案时间、每日用电脑时间方面差异无统计学意义(P>0.05)。在每周加班及熬夜时间、家族史、睡眠质量、睡眠时间、每日手持电子设备时间、枕头类型及头痛持续时间方面比较,差异有统计学差异(P<0.05)。颈源性头痛组患者中入睡困难、早睡、多梦的比例高于非颈源性头痛组(P<0.05);头痛持续0.5～2.0 h、 4～72 h、72 h以上、枕头类型的比例低于非颈源性头痛组(P<0.05)。睡眠质量、头痛持续时间、枕头类型与颈源性头痛独立相关(P<0.05)。结论:颈源性头痛患者与非颈源性头痛在每周加班及熬夜时间、家族史、睡眠质量、睡眠时间、每日手持电子设备时间、枕头类型及头痛持续时间方面存在差异,其中睡眠质量、头痛持续时间、枕头类型是颈源性头痛独立危险因素。

Objective:To investigate the differences between cervicogenic and non-cervicogenic headache,and to analyze the independent risk factors of cervicogenic headache.Methods:198 patients with cervicogenic headache and 80 patients with non-cervicogenic headache were analyzed for the difference.Multivariate Logistic regression was used to analyze the independent risk factors of cervicogenic headache.Results:There was no significant difference in age,gender,daily hours of working at desk,and daily hours of computer use between the patients with cervicogenic headache and those with non-cervicogenic headache(P>0.05),but there was a statistically significant difference in weekly overtime and staying up late during the night,family history,sleep quality,sleep time,daily time with electronic device in hands,pillow type,and duration of headache between the two groups(P<0.05).Patients with cervicogenic headache were more likely to have difficulty falling asleep,sleeping early,and dreamy sleep(P<0.05),but less likely to have headache lasting for 0.5-2.0 h,4-72 h,>72 h,and other pillow types(P<0.05),compared with patients with non-cervicogenic headache.Sleep quality,duration of headache,type of pillow were independently associated with cervicogenic headache(P<0.05).Conclusions:Patients with cervicogenic headache and non-cranial headache differ in weekly overtime and staying up late during the night,family history,sleep quality,sleep time,daily hours with electronic device in hands,type of pillow,and duration of headache.Among these,sleep quality,duration of headache and type of pillow are independent risk factors for cervicogenic headache.
