摘要
目的:探讨实时三维超声心动图(RT-3DE)技术评估原发性高血压病患者右心功能的应用价值。方法:选取2017年7月至2018年6月珠海市人民医院收治的原发性高血压患者119例,根据左室构型不同分为A组、B组、C组、D组,同期选取32例健康者作为对照组。均行RT-3DE技术检查,记录右心室收缩末容积(RVESV)、右心室舒张末容积(RVEDV)、右心室射血分数(RVEF);并以容积-时间曲线及定量组织速度成像(QTVI)对右心室侧壁三尖瓣环处舒张晚期峰值速度(Am)、舒张早期峰值速度(Em)进行测定,计算Em/Am。结果:5组患者Am、Em、Em/Am比较,差异有统计学意义(P<0.05);体部:B组EDV大于对照组和A组(P<0.05),D组EF小于对照组、A组和B组(P<0.05);流入道部:C组、D组EF小于对照组和A组(P<0.05),D组EDV、ESV大于对照组(P<0.05);整体:C组EF小于对照组和A组(P<0.05),D组EF小于对照组、A组和B组(P<0.05)。结论:RT-3DE技术可对原发性高血压患者右心形态、功能进行有效评估,对临床诊治具有重要意义。
Objective:To investigate the clinical use of real-time three-dimensional echocardiography(RT-3 DE)in evaluating right ventricular function in patients with essential hypertension.Methods:A total of 119 patients with essential hypertension admitted to Zhuhai Municipal People’s Hospital between July 2017 and June2018 were included in the study.All patients were divided into groups A,B,C and D according to left ventricular configuration.A contemporary cohort of 32 healthy subjects was included as the control group.The right ventricular end-systolic volume(RVESV),right ventricular end-diastolic volume(RVEDV)and right ventricular ejection fraction(RVEF)were recorded by RT-3 DE.The late diastolic peak velocity(Am)and early diastolic peak velocity(Em)at the tricuspid annulus of the right ventricular lateral wall were measured by volume-time curve and quantitative tissue velocity imaging(QTVI),and Em/Am was calculated.Results:There were statistically significant differences in Am,Em and Em/Am among the five groups(P<0.05).Body:The EDV in group B was larger than that in the control group and group A(P<0.05),and the EF in group D was lower than that in the control group,and groups A and B(P<0.05).Inflow tract:The EF in groups C and D was lower than that in the control group and group A(P<0.05),and the EDV and ESV in group D were larger than that in the control group(P<0.05).Overall:The EF in group C was lower than that in the control group and group A(P<0.05),and the EF in group D was lower than that in the control group and groups A and B.Conclusion:RT-3 DE may effectively evaluate the right ventricular configuration and function in patients with essential hypertension,which is of great significance for clinical diagnosis and treatment.
