慢性肾衰竭患者磁共振弥散加权成像检查图像分析

Magnetic resonance diffusion-weighted imaging study in patients with chronic renal failure

目的:观察慢性肾衰竭(CRF)患者磁共振弥散加权成像(DWI)检查的图像,探讨不同肾功能及不同期DWI改变。方法:选取2016年1月至2018年12月我院收治的CRF患者80例作为研究组,纳入同期健康体检者86例作为对照组,均接受DWI检查。统计两组表观扩散系数(ADC)值、研究组不同临床分期及不同b值下ADC值,并分析其影像学特征。结果:研究组髓质、皮质ADC值较对照组低(P<0.05);研究组Ⅰ期患者髓质、皮质ADC值高于Ⅱ期、Ⅲ期患者(P<0.05);b值=300 mm2、500 mm2、800 mm2时,研究组Ⅰ期患者ADC值高于Ⅱ期、Ⅲ期患者(P<0.05)。结论:DWI可通过ADC值量化CRF患者肾脏功能状态及反映病情严重程度,为临床后续治疗提供依据。

Objective:To investigate the changes in magnetic resonance(MR)diffusion-weighted imaging(DWI)findings in patients with different levels of renal functions and stages of chronic renal failure(CRF)through analysis of MR-DWI images.Methods:A total of 80 CRF patients admitted to our hospital between January 2016 and December 2018 were enrolled as the study group,and a contemporary cohort of 86 individuals who underwent health check-up in our hospital were included as the control group.All patients underwent DWI.The apparent diffusion coefficient(ADC)was recorded for the both group.Meanwhile,the study group was evaluated for clinical staging of CRF,ADC under different b values,and the imaging features.Results:The medulla and cortex ADC values in the study group were lower than those in the control group(P<0.05).In the study group,stage I patients showed higher medulla and cortical ADC values than did stages II and III patients(P<0.05);at b values of 300 mm2,500 mm2 and 800 mm2,the ADC values of stage I patients were higher than those of stages II and III patients,respectively(P<0.05).Conclusion:DWI may help quantify the renal function status and indicate the disease severity in CRF patients via ADC value,and therefore offer important reference for subsequent treatment in clinical practice.
