摘要 目的 探讨肺癌患者运动心肺功能的特点及其可能的影响因素。方法 对 2 0例健康者和 198例肺癌患者行静息肺功能、心电图和运动心肺功能测定。结果  1.与健康组相比 ,静息肺通气功能正常的肺癌患者 VO2 %P、 VO2 /kg、AT、 VO2 /HR %、 VE、VT/VC显著降低 ,而BR显著增加 (P <0 .0 5或P <0 .0 1)。 2 .在静息肺通气功能正常的情况下 ,周围型和中心型组间各指标均无显著性差异。 3 .肺癌患者心肺功能与TNM分期有密切关系 (P <0 .0 5或P <0 .0 1)。 4.血管侵犯组W %、 VO2 %P、AT、 VO2 /HR %较无血管侵犯组显著降低(P <0 .0 5或P <0 .0 1)。结论 肺癌患者在静息肺功能正常的情况下 ,存在着运动通气受限。 Objective To explore the characteristics of exercise cardio pulmonary function and its possible influencing factors in patients with lung cancer. Methods The pulmonary function, ECG and exercise cardio pulmonary function were measured in 198 patients with lung cancer and 20 healthy controls. Results 1. Compared with healthy group, O 2%P, O 2/kg, AT, O 2/HR%, E and VT/VC significantly decreased in lung cancer patients with normal resting pulmonary ventilation, however, BR remarkably increased (P< 0.05 or P<0.01). 2. In patients with normal resting pulmonary ventilation, there was no significant difference of exercise cardio pulmonary function between the central and peripheral lung cancer groups. 3. The exercise cardio pulmonary function was closely related to the TNM stages (P<0.05 or P<0.01). 4. W%, O 2%P , AT and O 2/HR% in patients with great vessel invasion were remarkably lower than those without great vessel invasion (P<0.05 or P<0.01). Conclusion The results suggest that exercise ventilation is impaired in lung cancer patients with normal resting ventilation. And the decrease of exercise cardio pulmonary function may be related to TNM stage and to great vessel involvement.
