摘要
目的 研究肺癌患者外周血细胞角蛋白 1 9(CyFRA2 1 1 )水平和CK1 9mRNA表达水平的相关性 ,并评价其对肺癌临床诊断的价值。方法 用逆转录多聚酶链反应法 (RT PCR)和酶联免疫吸附法 (ELISA)分别测定 91例肺癌、33例良性肺病患者、50例健康人外周血CK1 9mRNA的表达水平和血清CyFRA2 1 1水平。结果 肺癌组患者CK1 9mRNA阳性率和CyFRA2 1 1水平明显高于良性肺病组 (P <0 .0 1 )和健康人组 (P <0 .0 1 )。而且临床分期越晚CK1 9mRNA阳性率和CyFRA2 1 1水平越高 ,CK1 9mRNA对肺癌早期较为敏感。各组织类型间CK1 9mRNA阳性率无显著差异 (P >0 .0 5) ,而CyFRA2 1 1对鳞癌敏感性高于其他类型。结论 肺癌患者外周血细胞角蛋白 1 9(CyFRA2 1 1 )水平和CK1 9mRNA表达水平呈正相关性 。
Objective To evaluate the correlationship between CyFRA21 1 in serum and CK19mRNA in peripheral blood mononucleated cells(PBMCS) and their values in clinical diagnosis of lung cancer.Methods Expression of CK19mRNA in PBMCS were determined by RT PCR,the serum levels of CyFRA21 1 was determined by Enzyme Linked Immunosorbent Assay(ELISA) in 91 cases of lung cancer, 33 cases of benign lung diseases and 50 cases of healthy controls. Results There were a statistically significant differences in positive rates of both CK19mRNA expression and serum levels of CyFRA21 1 between lung cancer and benign lung disease(BLD) or healthy controls( P <0.01). There were positive correlationship between the positive rate of CK19mRNA and serum CyFRA21 1 level. The rate of CK19mRNA positivity was not statistically different in different histological subtypes( P >0.05),but there was higher CyFRA21 1 positive rate in squamous cell carcinoma(66.7%). The positive rate of CK19mRNA and serum levels of CyFRA21 1 in stage Ⅳ were higher than that in stage Ⅰ+Ⅱ( P <0.05). Combined detection of these two marker were more valuable than single item determination( P <0.05). Conclusion There were positive relationship between the positive rate of CK19mRNA and serum CyFRA21 1 level. These two markers can be used for early diagnosis and pathological typing of lung cancer.
