外科急症合并糖尿病的围手术期处理

Perioperative managment of acute surgical patients associated with diabetes mellitus

目的 探讨外科急诊合并糖尿病患者的围手术期处理。方法 Ⅰ组 1995～ 2 0 0 0年 78例行围手术期严格的血糖监测和治疗 ;Ⅱ组 1990～ 1994年 5 1例行常规方法对照 ,对两组病例进行回顾性分析。结果 I组 78例中死亡 3例 ,病死率 (3.8% ) ,并发症 12例 ;Ⅱ组 5 1例 ,死亡 6例 ,病死率 (11.8% ) ,并发症 15例。结论 外科急诊合并糖尿病的围手术期 ,正确使用胰岛素能有效减少并发症和死亡率。

Objective To study the perioperative management of acute surgical patients associated with diabetes mellitusMethods Retrospective analysis of clinical data of two groups of acute surgical patients associated with diabetes mellitus. The included in group Ⅱ were 78 cases subject to intravascularly regular insulin therapy according to blood glucose from 1995～2000 and those in group Ⅱ were 51 cases subject to routine therapy between 1991～1995 perioperatively.Results In the group Ⅰ 3 cases died ( 3.8 %) and complication occurred in 12 cases.In the group II 6 cases died ( 11.8 %) and complication occurred in 15 cases. There was significant difference between the two groups.Conclusion Intravenous regular insulin therapy according to blood glucose perioperatively was an effective method for acute surgical patients associated with diabetes mellitus
