颜面不对称畸形方丝弓矫治前后头影测量分析

Posteroanterior Cephalometric Study of the Patients with Facial Asymmetry Treated by Edgwise Appliance.

目的 :探讨颜面不对称畸形的形成机制及评价方丝弓矫治后的效果。方法 :收集 34例颜面不对称畸形患者行方丝弓矫治 ,并对其矫治前后进行头影测量分析。结果 :①颜面不对称畸形患者矫治前 ,面中下部各对称性标志线距对侧与偏斜侧比较均有显著或极显著性差异。②治疗后 ,左右各对称性线距差距明显缩小 ,具有极显著或高度显著性差异。③治疗后 ,面部横向宽度的改变 ,以上颌宽距增大最为显著 ,而下颌宽距则无显著性改变。④治疗后 ,正中标志均明显向中线聚拢 ,以颏下点最为明显。结论 :①颜面不对称畸形形成机制与偏斜侧上颌骨发育不足及下颌异常摆动旋转有关。

Objective:To investigate the pathology of facial asymmetry and the treatment effects of Edgwise appliance on it.Methods:34 patients with facial asymmetries were treated by Edgwise appliance.The posteroanterior cephalometric analysis of them were made to evaluate the effect before and after treatment.Results:The dimensions of J, A6, B6, AG to MSL of the health side of the patients with asymmetry before treatment were obviously wide than that of the oblique side ( P < 0.05 ,0.01,0.05.0.01).The two side leeway of J to MSL, A6 to MSL, B6 to MSL, AG to MSI were significantly different after treatment by Edgwise appliance ( P <0.05, 0.01, 0.01, 0.01, 0.01, 0.001). J(R) to J(L), A6(R) to A6(L), B6(R) to B6(L) were widen obviously after treatment ( P <0.05,0.01,0.05), but AG (R) to AG(L) had not obviously difference ( P >0.05). The dimensions of IPU, IS, Ii, IPL, Me to MSI redueced significantly after treatment ( P <0.05, 0.01, 0.001, 0.001, 0.001).Conclusions:The pathology of facial asymmetry maybe collective with the maxillary base bone of the oblique side developing insufficient and mandible rotating or oscilating unusual. Edgwise appliance was suitable for treatment of facial asymmetry.
