改良Millard法治疗123例单侧唇裂的临床研究

Primary Repair of Unilateral Cleft Lip with a Modified Millard Rotation Advancement Technique in 123 Cases.

目的 :Millard法是修复单侧唇裂一种较好的手术方法 ,但仍然存在着一些缺陷。为了改善上唇形态 ,延长唇高 ,本文对Millard法进行了改良 ,经 12 3例手术验证 3～ 6年随访 ,效果较好。方法 :在Millard法定点的基础上 ,在“3～ 5” ,“3～ 6”间定“9、10”点 ,使“3～ 9～ 10”为 2mm的等边三角形。在“4”点外侧 2mm处定“11”点 ,将三角瓣插入以延长唇高 ,口轮匝肌对位缝合。结果 :应用上述方法进行单侧唇裂的修复与经典的Millard相比 ,可以更好的改善鼻畸形 ,恢复上唇的丰满度 ,延长唇高 ,使上唇形态更自然 ,更符合美观和生理要求。结论 :在单侧唇裂修复术中 ,采用改良Millard法进行整复 。

Objective:Millard rotation-advancement technique is very useful in the repair of unilateral cleft lip cases,but it is not always satisfied in some cases.The goal of this article is to present a modified Millard technique for the repair of unilateral cleft lip.Methods:Over 6 years 123 new patients with unilateral cleft lip was treated by the author.All patients was operated according to the same surgical procedure.The modified point is that between the dot 3 and dot 5;dot 3 and dot 6,find out dot 9 and dot 10,so as to make 3～9～10 dots forming a equilateral triangle with 2mm every lateral.Then at the external side of dot 4,finding out a dot 11 and making the length of dot 4 to dot 11 is 2mm.After decision,the operation was finished.Results:Compared with traditional Millard technique,our modified method is better in the correction of the nosal deformity, lip lengthening and appearance of the face.Conclusions:The modified Millard technique is very useful in unilateral cleft lip repair.The repaired lip looks more beautiful.
