Window Techniques for Pulmonary Image in Chest CT

摘要 目的 研究胸部CT检查的最佳肺窗显示技术。方法 对 2 15例胸部CT检查病人 (包括肿块n =5 7,结节n=78,炎症n =3 8,其他病变n =42 )分别用①宽窗加图像边缘强化 (imageenhance ,简称IE)、②宽窗、③窄窗加IE、④窄窗四种窗技术显示 ,并进行比较分析 ;宽窗为W 160 0HU/L - 45 0HU ,窄窗为W 80 0HU/L - 65 0HU ;全部病例采用螺旋扫描 ,层厚 10mm ,Pitch 1~ 1.5 1。结果 肿块、结节、炎症病变均以宽窗加边缘强化为最佳显示 ,能充分显示病变的质感、边缘及钙化 ,立体感强 ;宽窗较窄窗或窄窗加IE在显示病灶质感、钙化、立体感方面好一些 ;窄窗或窄窗加IE能显示高反差肺纹理 ,但对多数病例无益于诊断。结论 宽窗加边缘强化是常规胸部CT检查的最佳肺窗技术 。 Objective To evaluate the best window display technique of pulmonary image.Methods A total of 215 cases(mass n=57;single/multiple nodules n=78;infection n=38;others n=42)underwent chest CT were performed and in using the following techniques respectively:① wide window plus image enhance(IE),② wide window, ③narrow window plus IE, ④narrow window. Wide window was W 1 600 HU/L-450 HU, and narrow window was W 800 HU/L-650 HU. All cases were scanned by helical CT(lamina 10 mm, Pitch 1~1.51). Results The optimal displaying in pulmonary images of masses, nodules, and inflammation were performed by using wide window plus IE, which demonstrated sharp margins, quality characters, calcification and stereopsis of all lesions. The effect of simple wide window on these aspects was superior than narrow window or narrow window plus IE. The hyper-contrast pulmonary markings were revealed by narrow window images or narrow window plus IE, but they were difficult to make correct diagnosis in most cases. Conclusion The wide window plus IE is the best window technique for scanning the pulmonary images in conventional chest CT studies and this technique will significantly improves the diagnostic accuracy.
