儿童视神经胶质瘤

Optic gliomas in children

目的总结儿童视神经胶质瘤的临床特点和治疗经验。方法回顾分析过去7年间我科收治的18例儿童视神经胶质瘤的临床资料。结果本组18例,占同期住院儿童颅内肿瘤的1.0%,发病平均年龄为7.9岁,男女之比为5∶4。根据术前临床和影像表现将其分为3类:肿瘤为视神经型者占16.7%;弥漫性视交叉型占11.1%;视交叉-下丘脑外生型者占72.2%。显微手术全切和近全切除率达88.9%,术后尿崩及电解质紊乱(38.9%)较多见。结论儿童视神经胶质瘤为良性肿瘤,预后较好,手术切除和及时处理术后并发症、恰当的放疗是治疗的关键。

Objective To summarize the clinical characteristics and therapeutic experience of optic gliomas in children.Method The clinical data of18patients of our department in the past7years were analyzed.Result In this series,childhood optic gliomas represented1.0%of all intracranial tumors in children,the average age was7.9years,the ratio of boys to girls was5:4.Based on the preoperative manifestation and imaging,the tumors were classified into three types:the optic nerve type(16.7%),the diffuse chiasm type(11.1%),and the chiasm-hypothalamustype(72.2%).Almost88.9%of the tumors was totally or sub-totallyremoved.Diabetes insipidus and electrolyte disturbances(63.6%)were the most common postoperative complications.Conclusion The optic glioma is a benign intracranial tumor with good prognosis.The key point of treatment is surgical resection combined with proper postoperative radiotherapy.
