nm23 mRNA、CD44s和CD44v6在胃腺癌组织中的表达及其意义

Expression and significance of nm23 mRNA, CD44s and CD44v6 in human gastric adenocarcinoma

目的 探讨nm2 3mRNA、CD4 4s和CD4 4v6在胃腺癌组织中的表达及其与胃腺癌病理学分级、浸润程度和转移之间的相关性。方法 应用免疫组织化学和mRNA原位杂交方法检测了 6 2例胃组织标本中nm2 3mRNA、CD4 4s和CD4 4v6的表达情况。结果 nm2 3mRNA在胃正常组织和癌组织中的表达无显著性差异 (P >0 .0 5 ) ,且与胃腺癌浸润和病理学分级无相关性 (P >0 .0 5 ) ,但与胃腺癌淋巴结转移呈明显负相关 (r =- 4 .93)。CD4 4s在胃腺癌与胃正常组织中的阳性表达率分别为 4 8% (19 4 0 )和 13.6 % (3 2 2 ) ,有显著性差异 (P <0 .0 1)。CD4 4v6只在胃腺癌中表达 ,阳性率为 6 3.3% (2 5 4 0 ) ,同时CD4 4v6表达与胃腺癌分化程度、淋巴结转移及其浸润程度呈明显正相关 (r =5 .0 4 )。结论 CD4 4v6高表达和nm2 3mRNA低表达患者淋巴结转移率高。联合检测CD4 4v6和nm2 3mRNA能更准确地判断胃腺癌的转移状态。CD4 4v6过量表达与胃腺癌的分化程度及浸润相关。

Objective To investigate the relationship between the expression level of nm23 mRNA, CD44s and CD44v6 and the pathological grade, invasion, and lymph node metastasis of human gastric adenocarcinoma.Methods The expression of nm23 mRNA, CD44s and CD44v6 in 62 human gastric adenocarcinoma were detected by using immunohistochemical (IHC) staining and in situ hybridization (ISH). Results The expression of nm23 mRNA in the tissues of gastric adenocarcinoma and normal gastric mucosa had no significant difference ( P >0.05), and did not correlate with the invasion of tumor and pathological grade of gastric adenocarcinoma ( P >0.05). However, the expression of nm23 mRNA was associated with the lymph node metastasis of gastric adenocarcinoma negatively ( r =-4.93). The expression of CD44s in the tissues of gastric adenocarcinoma and normal tissues of gastric mucosa were 48% (19/40) and 13.6 (3/22), respectively, and had significant difference ( P <0.01). But CD44v6 was expressed in the tissues of gastric adenocarcinoma only, and the positive rate was 63.3%(25/40), which was not only significantly associated with the lymph node metastasis and invasion of the tumor, but also associated with pathological grade of the tumor ( r =5.04).Conclusion The patients who had overexpression of CD44v6 and low expression of nm23 mRNA had a higher metastatic rate of lymph node. Combined detection of the expressions of nm23 mRNA and CD44v6 is a reliable index to evaluate the matastasis of gastric adenocarcinoma. Overexpression of CD44v6 is correlated with invasion of the tumor and pathological grade of gastric adenocarcinoma.
