摘要
目的:应用声学定量(AQ)技术评价不同左室几何构型高血压(EH)患者的左室收缩和舒张功能。方法:对51例EH患者和20例正常对照进行了AQ检测,根据左室重量指数和相对室壁厚度,EH组又分为左室几何构型正常组、左室向心性重构组和左室向心性肥厚组。结果:左室几何构型正常组左室舒张功能无明显变化,而左室容量负荷出现增大,进一步引起早期左室泵功能的增强;向心性重构组患者被动充盈减弱,主动充盈增强,左室舒张功能明显减退;左室向心性肥厚型者随着左室肥厚的发展,左室收缩和舒张功能均显著减退。结论:EH患者左室几何构型对心功能产生不同的影响,应对症治疗。
Objective:To evaluate left ventricular systolic and diastolic function in patients with hypertension by acoustic quantification(AQ)technique.Methods:51patients with hypertension and20control subjects were studied by AQ technique.The hypertension group was divided into three sub-groups by left ventricular mass index and relative wall thickness:normal left ventricular geometry,con-centric remolding and concentric hypertrophy subgroup.Results:Compared with control subjects,pa-tients in normal left ventricular geometry subgroup had normal left ventricular diastolic function,but left ventricular systolic function increased.Left ventricular diastolic function significantly decreased in patients of concentric remolding subgroup,but diastolic function was normal.Both left ventricular diastolic and systolic functions were greatly lower in patients of concentric hypertrophy subgroup.Conclusion:Patients with difference geometry hypertension had different left ventricular function.
