摘要
目的 :观察局部射频透热联合化疗治疗晚期消化道恶性肿瘤的临床疗效及其副反应。方法 :82例晚期消化道恶性肿瘤被随机分为单纯化疗组及射频透热化疗组各 4 1例。单纯化疗组全身采用以 5 Fu 35 0mg/m2 /d ,持续 8～ 2 4小时 ,连用 2 8天 ,DDP 3 5～ 7 5mg/m2 /d ,每周用 4天 ,连用 4周 ,原发性 (或转移性 )肝癌、胰腺癌、腹腔较大肿块的行区域性动脉置管 ,用动脉泵给予 5 FU。射频热化疗组采用 :西咪替丁 2 0 0mg ,地塞米松 10mg ,利多卡因 10mg ,顺铂 2 0mg加入 5 0℃生理盐水 2 0 0 0～ 2 5 0 0ml行腹腔灌注 ,于灌注后 30分钟内进行射频热疗 6 0～ 90分钟 ,每周 1次 ,热疗次数为 8～ 10次。射频透热化疗组全身化疗方法同单纯化疗组 (5 Fu 35 0mg/m2 /d ,持续 8～ 2 4小时 ,连用 2 8天 ,DDP 3 5～ 7 5mg/m2 /d ,每周用 4天 ,连用4周 ,原发性或转移性肝癌、胰腺癌、腹腔较大肿块的行区域性动脉置管 ,用动脉泵给予 5 Fu)。结果 :治疗组总有效率 (CR +PR) 75 6 % ,而对照组总有效率 5 3 6 6 %。两组有显著性差异 (P <0 0 5 )。两组化疗副反应发生率无显著性差异。结论 :射频透热化疗联合全身化疗对晚期消化系统肿瘤可取得较好的近期疗效 。
Objective:To observe the overall response and side effect to the radio chemotherapy in advanced gastrointestinal cancer.Methods:The eighty two patients were randomly divided into two groups: sole chemotherapy group and radio chemotherapy group. The control group was treated with continues infusion 5 Fu 350mg/m 2/day for 8-24 hour of twenty eight days, and DDP 3 5-7 5/m 2/day infused for four times of one week for 4 weeks. 5 Fu was perfused to indwelling regional arterial A port in patients with primary hepatocellular cancer\metastatic hepatic cancer\pancreatic cancer and abdominal big mass. After perfusion of abdominal cavity with cimetidine 200mg, dexamethasore 10 mg, lidocaine 10mg, DDP 20mg, normal saline solution (50℃) 2000～2500 ml, the radio chemotherapy of treatment group was performed 60～90 min once a week for 8-10 times. Systemic chemotherapy in treatment group was continuous infusion 5 Fu (as to as control group) and DDP.Results:The effective rate in treatment group was 75 6%, control group was 53 66%, there was a difference( P <0 05) among them. Side effects of chemotherapy between them are no difference.Conclusion:radio chemotherapy combination systemic chemotherapy can reach a higher response rate in patients with advanced gastrointestinal trace cancer, with mild side effects.
