白血病胸部浸润的临床X线表现(附68例分析)

目的 :总结白血病在胸部不同组织、器官浸润的临床X线表现 ,旨在提高对白血病胸部浸润的医学影像诊断水平。方法 :随机收集不同类型、不同病期白血病患者胸部X线资料 10 4例 ,其中 6 8例具有胸部异常改变 ,部分患者有不同病期及治疗过程中的X线胸部平片用以对照观察。本文就其X线征象及病理基础进行分析讨论。结果 :6 8例具有胸部异常的患者中 ,其X线表现形式多种多样 ,同一患者可有多种异常表现形式同时存在。其中 ,肺间质性改变 4 9例 ,肺粟粒样病灶 2 1例 ,肺结节影 5例 ,肺内片絮状影 15例 ,纵隔、肺门、腋窝淋巴结肿大 2 4例 ,心影增大 12例 ,胸膜受累 6例 ,乳腺受累 3例 ,骨性结构受累 1例。结论 :白血病可累及胸部所有脏器或组织 ,其临床X线表现形式多种多样 ,如伴有肺感染、肺出血等疾患 ,其X线征象将更趋复杂。

Shandong Provincial Medical Imaging Research Institute,Jinan 250021, China AbstractObjective:To analysis the clinical X-ray findings of thoracic different tissue and organs in leukemic infiltration,so as to improve the diagnostic level of leukemic infiltration in thorax.[WTMethods:Collect thoracic X-ray films 104 cases randomly,they were difference courses of leukemic patients,among them, abnormal thoracic changes were found in 86 cases,the X-ray findings and pathological basis were analysed and discussed.Results:Among 68 thoracic abnormal cases,the lung markings increased were 49 cases,military focuses of the pulmonary were 21 cases,the pulmonary tubercles were 5 cases,pulmonary patchy shadow were 15 cases,lymphadenopathy of mediastinum?hilum and axilla were 24 cases,cardiac shadow enlargement were 12 cases,pleural abnormality were 6 cases,breast abnormality were 3 cases,skeletal abnormality was 1 case.5'HZConclusion:All the thoracic organs and tissue can be invaded by leukemia,and the clinical X-ray findings are varied.The X-ray manifestation can be more complicated if the pulmonary infection and hemohrrage etc are complicated.
