血清NSE对小细胞肺癌的诊断价值 被引量:2

摘要 采用BAS-ELISA法测定了58例正常对照(NC)、17例肺良性疾患(BPD)、16例小细胞肺癌(SCLC)和29例非小细胞肺癌(NSCLC)患者血清神经元特异性烯醇化酶(NSE)水平,并和脂质结合唾液酸(LSA)、癌胚抗原(CEA)作对比研究。结果表明SCLC患者血清NSE水平明显升高,其升高程度与肿瘤负荷有关,随着肿瘤转移数的增加而增加,诊断阳性率达81.3%(13/16),显著高于NSCLC(6.9%)和BPD组(5.9%),特异性93%,诊断准确率为88.9%;而血清LSA、CEA阳性率分别仅为37.5%(6/16)和18.8(3/16)。可见,NSE在SCLC诊断、鉴别诊断和病情判断方面具有更重要的价值。三者联合检测可使阳性率由单测NSE的81.3%上升到93.8%。 The serum neuron specific enolase (NSE), lipid-pound sialic add (LSA) and carcinoembryonic antigen (CEA) levels were measured in 58 normal controls, 17 patients with benign pulmonary diseases (BPD), 16 patients with small cell lung cancer (SCLQ and 29 patients with non-SCLC. The results showed that serum NSE level was markedly elevated in patients with SCLC compared to non-SCLC and BPD. NSE levels at diagnosis tended to correlate with the initial tumor burden as evidenced by increasing mean NSE value be ing associated with increasing number of metastatic sites. Thirteen out of sixteen SCLC patients had NSE level raised but only six had LSA raised (sensitivity 37.5%) and three CEA raised (sensitivity 18.8%). Our study indicated that NSE was more helpful than LSA, CEA for diagnosis and differential diagnosis of SCLC and evaluating status of the disease. The incidence of at least 1 elevated marker increased from 81.3% using NSE alone to 93.8% using NSE, LSA and CEA together.
