氟碳乳剂对大鼠离体心脏缺血心肌的保护作用

摘要 作者在离体大鼠工作心脏模型上观察了国产Ⅲ型氟碳乳剂(PFCE-Ⅲ)与晶体停搏液对缺血心肌的保护作用。实验条件:在15℃下,心脏缺血停搏30min;在37℃下,再灌30min。晶体停搏液组与PFCE-Ⅲ组再灌注30min时,心脏血液动力学参数(CHI)的恢复率分别为:LVSP,86±8%与100±8%;dp/dt_(max),92±8%与111±16%;-dp/dt_(max),68±10%与105±17%;LVEDP,171±36%与112±8%;AoF,60±12%与93±11%,有显著差异(P<0.01)。电镜下PFCE-Ⅲ组的心肌缺血性损害较晶体停搏液组轻。结果表明:PFCE-Ⅲ对缺血心肌的保护效果比晶体停搏液好,这可能与氧溶解度高的PFCE能向缺血心肌提供更多的氧有关。 Myocardial protective effect of oxygenated perfluorocarbon emulsions (PFCE) with high oxygen solubility, was compared with that of crystalloid cardioplegia with an isolated working rat heart model. The hearts underwent 30 minutes of ischemic cardiac arrest at 15 t , followed by 30 minutes of reperfiision at 37 C . The recovery of cardiac hemodynamic indices at 30 minutes of reperfusion for crystalloid group vs PFCE group was 86±8% vs 100±8% (P<0.01 )for LVSP;92±8% vs 111 ± 16% (P<0.05) for dp/dtmax;68±10% vs 105±17% (P<0.01) for dp/dtmax 171 ±36% vs 112±18% (P<0.01) for LVEDP;60±12% vs 93±11% (P<0.01 )for AoF .The ultrastructural damage was less common and extensive in PFCE group than in crystalloid group. The results clearly showed that PFCE could protect ischemic heart better than crystalloid cardioplegia.
