头颈部恶性肿瘤患者治疗前后的生活质量比较研究

A study on the quality of patients with life of head and neck tumor pre-and post-treatment

目的 了解头颈部肿瘤患者的生活质量特点 ,并研究不同治疗方法对患者生活质量的影响。方法 用WHO QOL1 0 0评估了 566名经病理检验证实为头颈部恶性肿瘤的住院患者治疗前和治疗后的生活质量。结果 1 .治疗前患者的生活质量低于正常对照 ;2 .治疗后患者的生活质量与治疗前相比进一步下降 ;3 .放疗后患者生活质量受损的程度和范围比手术治疗后更为明显 ,尤其是心理功能的进一步损害值得临床医生关注和干预。结论 头颈部恶性肿瘤和治疗手段对生活质量都有影响 ,放疗的损害重于手术治疗。

Objectives To understand the quality of life of head & neck tumor patients and evaluate the impact of different treatment approaches on quality of life. Methods We evaluated 566 head and neck malignant tumor patients using WHO QOL100 before and after treatment. The diagnoses were made based on pathological examination. Results 1. Quality of life of the patients was poorer than normal control at baseline. 2. Most aspects of the quality of life continued declined following treatment. 3. The impairment on quality of life were much more significant in the patients who received radiation therapy than in the patients who received surgical therapy, especially in the psychological function domain of patients. Conclusions Both the tumor and the treatment can damage the quality of life of the patients. The damage caused by radiation therapy was more significant than that caused by surgical therapy. Psychological intervention is crucial.
