自身免疫性甲状腺疾病尿液Tamm-Horsfall蛋白的变化

Changes of urinary Tamm-Horsfall protein level in patients with autoimmune thyroid disease

作者采用放免分析法检测了自身免疫性甲状腺疾病(AITD)72例患者尿中Tamm-Horsfall(TH)蛋白(THP)的含量,并与非自身免疫性甲状腺疾病(NAITD)患者及正常人作对照.结果表明:病程在1年以上的甲状腺机能亢进(35例)和甲状腺机能减退(24例)患者尿THP含量分别为5.64±5.26mg/L和3.33±2.80mg/L,均明显低于NAITD患者和正常对照组(分别为20.03±8.06 mg/L和28.96±16.68 mg/L).AITD患者尿THP含量与病程呈显著负相关(r=-0.539,P<0.01),与甲状腺功能状态和甲状腺自身抗体TGAb,TMAb无显著相关.提示:AITD患者可伴有肾脏远曲小管细胞的免疫损伤.检测尿THP含量可早期发现AITD患者的肾脏损害.

The concentrations of Tamm-Horsfall protein (THP)in urine from 72 patients with autoimmune thyroid disease (AITD), 13 patients with non-AITD and 30 healthy were measured by RIA. The results revealed that THP excretion of 35 patients with hyperthyroidism (5.64±5.26 mg/L) and 24 patients with hypothyroidism (3-33 + 2-80 mg/L)(the duration of disease more than one year) were significantly lower than those of non-AITD (20.03 ± 8.06 mg/L) and normals (28.98±16.68 mg/L). There is a negative correlation between THP level and duration of disease. The THP excretion was not correlated with thyroid function and patients with AITD may complicated with the immunological damage of the distal convoluted tubule of kidney. The detection of the THP concentration in urine may discover early damage of kidney.
