直肠癌卵巢转移26例诊治分析

Rectal Carcinoma with Metastasis to the Ovary

目的:探讨直肠癌卵巢转移的治疗方法。方法:分析1990～1999年26例直肠癌卵巢转移临床资料。结果:直肠癌卵巢转移好发于年青女性,以双侧多见,原发灶大多为低分化腺癌或粘液腺癌,多侵犯浆膜,临床分期多为Dukes C期以上,发生卵巢转移者预后较差。结论:女性直肠癌者应注意有无卵巢转移,对有卵巢转移者应在根治性切除原发灶的同时,一并切除子宫及双侧附件,对无卵巢转移者是否预防性切除卵巢应采取个体化原则。

Objective:To study the treatment of ovarian metastasis from rectum career. Methods:We review our 26 rectum cancer patients with metastasis to the ovary between 1990-1999. Results:Overian metastasis are often found bilateral,especially in premenopausal stage. Low differentiated or ring cell adenocarcinoma, tumors at advanced stage(Dukes C or up) are prone to metastaxize to ovary,and the patient' s prognosis is usually poor. Conculusions: It is mandatory to inspect the ovary during a laparotomy of rectal cancer woman. Patients with o-varian metastases should undergo additional uterectomy plus bilateral oophprectomy.
